The Family Health Centers located in Bellaire and East Jordan, combine federal assistance, patient fees, community resources and your support to offer quality, affordable medical services. This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n). East Jordan Family Health Center is a Federal Tort Claims Act (FTCA) deemed facility. Our Services

3D Mammography and X-ray
Obstetrics
Occupational Medicine
Gynecology
Pharmacy
Laboratory Services On Site (Provided by LabCorp)
Immunizations
Men’s Health
Adult Physicals and Well Child Exams
Flu Clinics
Prescription Assistance Services
Sliding Fee Discount Program

It’s about what becomes possible because of it.
As the Health Center expanded, patients began to experience something different, care that was more connected, more coordinated, and closer to home.

Primary care, pharmacy, lab, behavioral health, and support services began working together in ways that made care simpler and more complete.

For patients, that meant fewer barriers.

BELLAIRE DAM: IN CASE OF EVACUATION

In an abundance of caution, plans are in place to open evacuation centers. IF the dam safety protocols for the Bellaire Dam trigger an evacuation, two centers in Bellaire will open:
• Commission on Aging (COA): 308 E. Cayuga St.
• Bellaire High School: 204 W. Forest Home Ave.

Growth didn’t just happen across communities, it happened at home.

In 2011, the East Jordan location expanded to better serve a growing number of patients and services. The addition brought more exam rooms, improved patient flow, and integrated services like pharmacy and lab into one location.

It changed how care was delivered.

Patients could receive more of what they needed in one place.
Teams could work more closely together.
Care became more connected, and more convenient.

Sometimes growth isn’t about adding locations.
