Eastside Care AFH is a placement alternative for people with disability or older adult who needs technical nursing care, continuous supervision, or support in home environment. We work with the residents and their family to create an individualized care plan that supports each resident’s physical, social, emotional, and spiritual needs. We ensure that all our resident’s enjoy the highest quality of life while they cared in our home to maintain a happy and healthy lifestyle. We are located in a quiet and secure neighborhood that allows residents to feel safe indoors and outdoors. We are close to Crossroad and Overlake Shopping Center and Entertainment Centers.
