After tear trough filler, PRP injections, and Tox, this woman developed festoons and looked worse. She came to me for help and I dissolved out her filler then performed fat repositioning blepharoplasty. We opted to work with her own eyelid tissues for a natural improvement, rather than add fat or implants.

A lesson in subtlety. This patient with a keen eye wanted 1. to show less teeth with jaws relaxed, 2. to have equal upper and lower lip height ratio, 3. to make the smaller left upper lip the same size as the right upper lip, 4. to turn out the corners of the upper lips. We accomplished this with dermal filler. Excise lighting differences (bottom after photo is her selfie). Fordyce (white bumps) are unchanged from baseline.

The after photo is a selfie he took just 6 days out from filler to the tear trough and midface. There is definitely a place for dermal fillers when done correctly

It’s hard to improve upon a young and attractive face. This woman had been bothered by multiple subtle issues for a decade but did not want to drastically alter her appearance. We agreed upon a customized endoscopic browlift, upper eyelid blepharoplasty and ptosis surgery, lower fat repositioning blepharoplasty, and submental liposuction. Wow!

*LENGTHY BUT ILLUMINATING EXPLANATION* This lovely mature woman sought surgery to address tired looking eyes and forehead wrinkles from constantly (subconsciously) raising her brows to lift the heavy sagging eyelid skin. I performed browlift, upper blepharoplasty, and lower eyelid fat repositioning. Now her eyes look simultaneously brighter and softer. Browlift surgery lowered her hairline to restore facial balance and allowed her brows to remain in good position even after sagging eyelid skin removal stopped her subconscious need to lift her brows. Now her forehead wrinkles are also improved!

I sometimes hear “Am I too old for cosmetic eyelid surgery?” This 76 year old patient underwent in-office cosmetic blepharoplasty, ptosis surgery, and internal browpexy with oral sedatives and local anesthesia. She did great and loves her results! There is no age cut off for surgery. But the surgery plan must be tailored to each individual’s medical health and goals.
