Or order a calendar simply because our therapy dogs are so adorable. 🐾 So many programs at Partners In Care continue thanks to generous donors, with just a few examples below:

⭐ Transitions: a non-medical, resource-based program for people with life-limiting illnesses, where we help them or their loved ones navigate care, get connected with meals if there is food insecurity, connect with various housing resources, transportation, and more. We help them live with a better quality of life and are a life-line of support so no one falls through the cracks.

⭐ Grief support programs: free bereavement programs available to the community for those who have experienced the death of a loved one. A new program this year thanks to a generous donor was for parents experiencing the loss of a child.

⭐ Veterans outreach: we connect aging veterans with community resources, VA benefits, healthcare, and many other programs across Central Oregon to combat isolation and ensure veterans receive the unique care and recognition they deserve.

We celebrated 45 years as a hospice organization in Central Oregon, 20 years of our free Transitions program, and 10 years of improving quality of life with our palliative care team.

We had a fantastic time supporting Home Instead Be A Santa to a Senior program this year to bring joy throughout Central Oregon. Even one of our hospice nurses, Dan Jeans, got into the action at the present wrapping party, dressing up as Santa!

We are grateful for partners in the community like Home Instead and the many other organizations that show up each year to bring a smile to so many older adults this holiday season.

We are a proud supporter of Be A Santa to a Senior through Home Instead! We hope to see you at the wrapping party Wednesday night.

The Be a Santa to a Senior wrapping party is on Wednesday! This fun and festive event is open to the public at Silver Moon Brewing. Come by to wrap presents for local Meals on Wheels participants and enjoy food and festivites. It's a great family event to get into the holiday spirit. The BASTAS program is organized by Home Instead.

This Giving Tuesday, we ask that you support our team and the patients and families we serve. Each year, thousands of our neighbors across Central Oregon entrust is with caring for their loved ones, friends, and family with serious, chronic, and terminal illnesses.

Deeply rooted in the community and trusted by our neighbors, partners, and healthcare professionals, our team members and volunteers are honored to provide exemplary and patient-centered care every day. We couldn't do what we do without the support of the community. As a community based nonprofit organization, many of the services we provide free of charge, simply because it's the right thing... some of those include our Transitions program for those with life limiting illnesses, our community grief support groups, our Veterans outreach support, and more. Our team members go above and beyond to help out their most vulnerable neighbors to help improve quality of life and provide an extra layer of support for loved ones during challenging times.

If you can donate today, we ask that you not do it for the incentives (while amazing this week!), but for your neighbors across Central Oregon who may be facing health challenges and could use a compassionate hand to help them navigate their care.

We are so grateful The Bulletin interviewed one of our hospice nurses, Susan Myers, who was able to share both her personal and professional experience of caring for those with Alzheimer's and Dementia.

“I can’t emphasize enough that people with Alzheimer’s or any kind of dementia, they are still who they are in their core, they are there,” Susan Myers said.

Compassionate care is at the heart of Myers’ care for memory loss patients.

“I think they’re lovely human beings whose brains just don’t work the way they used to,” Myers said. “Understand that every human being at their core understands a compassionate approach.”

For the third year in a row, Partners In Care has been recognized with a prestigious five-star rating on the Consumer Assessment of Healthcare Providers and Systems (CAHPS) Hospice Survey. The Home Health team at Partners In Care also achieved a five-star rating from the Home Health Consumer Assessment of Healthcare Providers and Systems (HHCAHPS) Home Health Survey from the Centers for Medicare & Medicaid Services (CMS).

“We are very proud of our team and the exemplary care that our clinicians provide every day to our patients, their family members, and caregivers,” said Kristi Williams, RN, Quality Program Manager at Partners In Care. “This dual recognition for hospice and home health is a testament to the strong and patient-focused team we have here at Partners In Care.
