Today, we celebrate Rural Health Day and the vital role Community Health Centers (CHCs) play in keeping rural America healthy.

Community Health Centers proudly serve up to 1 in 3 rural Americans, ensuring access to affordable, comprehensive, and effective care close to home.

In communities where hospitals are closing at unprecedented rates, CHCs provide stability — bridging gaps in care at a fraction of the cost and delivering outcomes that often match or exceed national benchmarks in chronic disease management, prenatal care, and preventive screenings.

Rural communities deserve strong, sustainable healthcare. Community Health Centers make that possible.

Amid growing financial pressure and rapid change across the health care landscape, NACHC has convened a brain trust of 300 primary care leaders in Minneapolis this week to discuss bold strategies to turbocharge America’s health system by strengthening and expanding the nation’s largest primary care provider: Community Health Centers (CHCs).

The 2025 NACHC Partner Conference and Strategy Meeting gathers state/regional Primary Care Associations (PCAs), Health Center Controlled Networks (HCCNs), and National Technical Assistance Partners (NTAPs) to address emerging issues in community health and develop collaborative solutions. The event advances NACHC’s goal of positioning CHCs as the employers, providers, and partners of choice for the up to 1 in 7 patients they serve, the 330,000-strong workforce they employ, and the communities they impact through more than 17,000 locations.

This year NACHC has invited the American Academy of Family Physicians (AAFP), led by Executive Vice President and CEO, Shawn Martin, to promote closer collaboration between the state AAFP chapters and the PCAs around promising care models and advocacy. Anchoring the conversation about how to expand CHC patients served and address the crises in primary care access and chronic disease rates is Dr. Asaf Bitton, executive director of Ariadne Labs. Dr. Bitton is joining the Strategy Meeting to rally attendees in support of the “Triple Double,” which sets audacious but achievable goals to transform American primary care by 2030.

NACHC and eClinicalWorks have launched a new multi-year partnership to accelerate the adoption of health IT and AI solutions across Community Health Centers (CHCs). The goal: reduce administrative burden, strengthen workforce recruitment and retention, improve patient experience, and help CHCs remain both high-trust and high-tech.

“Health professionals will not be replaced by AI, but health professionals that don’t use AI will be replaced by those who do. This partnership is an important step toward ensuring CHCs, their workforce, and their patients are not left behind in the age of AI,” said Kyu Rhee, MD, MPP, President and CEO of NACHC. “From accelerating the use of ambient and predictive AI to streamlining clinical documentation and operations to the upcoming challenges posed by Medicaid eligibility changes, CHCs will increasingly rely on field-tested AI solutions like those offered by eClinicalWorks. Embracing technology and AI so that CHCs are not only high-trust, but also high-tech is critical as CHCs are employers, providers, and partners of choice across the health system."

Puerto Rico’s Community Health Centers (CHCs) continue to deliver consistent, comprehensive, and high-value care despite extraordinary and ongoing challenges. Chronic funding shortfalls, aging infrastructure, severe weather threats, and workforce shortages have pushed these health centers to innovate out of necessity — and they continue to meet rising community needs.

A new report co-authored by Peter Shin, Chief Science Officer, NACHC, Dr. John W. Hatch Center for Science; Darielys Cordero, Executive Director, Asociación de Salud Primaria de Puerto Rico; Roberto Ramirez, Professor (Ret.), Universidad de Puerto Rico - UPR; Paloma Hernandez, Chief Executive Officer, Urban Health Plan, Inc., NACHC Board Chair, highlights how Puerto Rico’s CHCs are adapting, strengthening operations, and finding new ways to serve patients amid constant disruption.

Their experience offers important insights for CHCs across the United States, especially in areas where climate risks, economic pressures, and provider shortages intersect.
