Most people don't miss the prevention window on purpose. They miss it because nothing felt urgent enough to act on.

Midlife is when that changes. Not because something is wrong, but because the margin between proactive and reactive starts to narrow.

The patients I see who age well aren't the ones who reacted fastest when something went wrong. They're the ones who stayed consistent before anything did.

A woman in the audience at Sunday’s M Factor 2 screening asked a question that stopped me.

“What do I need to tell my doctor to get them to help me?”

The fact that she had to ask that is worth sitting with.

Women navigating perimenopause should not need to arrive at a medical appointment with a prepared argument. They should not have to find the right language to be believed. A physician who knows your history, your patterns, and your baseline does not need to be convinced.

That is what this conversation is about. And it is why it matters that it is finally happening at scale.

The decade between 45 and 55 is when everything quietly shifts.

Bone density. Muscle mass. Cardiovascular risk. Metabolic health. All of it starts moving in the wrong direction if no one is paying attention.

Not because something is wrong with you. Because no one warned you this was coming.

This is the window. And it matters what you do with it.

The FDA approved a higher dose of semaglutide (Wegovy) this week, and I want to give you the clinical context, not just the headline.

The 7.2 mg dose showed greater weight loss than previous doses and is approved for adults with obesity or overweight with related health conditions.

GLP-1 medications are becoming an important part of metabolic care. But more is not automatically better. The right dose depends on your individual profile, your history, and what you are actually trying to accomplish.

Medication is one piece. Nutrition, lifestyle, and a physician who understands the full picture are the others.

A lifetime of watching medicine done right.

The physicians who shaped me never treated their patients as appointments to get through. They carried people. Quietly, without recognition, often at the expense of their own time and health.

Today we honor the physicians who sacrifice their time, energy, and often their own well-being in service of others. Caring for a community is not a job description. It is a commitment most people never see.

To my colleagues, my friends, and my family of physicians: this day is for you.

Last night’s M Factor 2 screening brought together a room full of women asking questions they have waited years to ask.

Perimenopause is not just about hot flashes. It affects brain health, cardiovascular function, metabolism, and sleep. Most women experiencing these changes have no idea they are connected.

What I hear most often is not frustration with symptoms. It is the feeling that no one has taken the time to connect the dots.

That conversation is finally happening at a larger scale. I was honored to be in that room, and to have BMPC there as well.

If you are navigating midlife health and looking for a physician who looks at the full picture, book a conversation at BethesdaDoctor.com

Thank you to and for bringing this conversation to Bethesda, and to my fellow panelists and for a thoughtful and necessary discussion.

Many patients assume feeling rushed is just part of seeing a doctor.

It shouldn’t be.

Complex health concerns do not fit neatly into brief visits. Hormones, cardiovascular risk, metabolic health, sleep, stress. These require time and context.

If you have ever left an appointment thinking, “I didn’t get to ask everything,” that is not a personal failure. It is a structural one.

Medicine works best when there is space for listening.

Direct Primary Care allows for longer visits, direct communication, and long-term prevention planning.

Save this if you’ve ever felt rushed in healthcare.

And if you’re looking for a more intentional approach to primary care in Bethesda, schedule a discovery call.
