There’s a shift happening with younger patients—especially in their 30s. 👀

More and more people are asking about fat grafting instead of fillers for volume restoration.

But is it actually the better option for younger faces?

Let’s talk about what’s driving this trend—and what people should consider before choosing either.

Excited to join an incredible international faculty at the NYC Facelift & Rhinoplasty Master Course this April.

I’ll be lecturing on deep plane facelift, structural neck contouring, and submandibular gland modification, sharing anatomy-based techniques for facial rejuvenation.

Looking forward to connecting with colleagues and advancing the field together.

Her comprehensive treatment plan included:

The Beauty of Saying No — Episode 3
A 25-year-old patient came in requesting a lip lift.

After a thorough evaluation, it was clear that surgery would do more harm than good in her case.

As a surgeon, it’s not just about knowing how to operate — it’s about knowing when not to. Every recommendation should be guided by anatomy, long-term balance, and the patient’s best interest.

Sometimes the most ethical decision is saying no.

Is 46 too young for a facelift? Not always.

At this age, it’s not about overdoing—it’s about doing what’s needed.
Her goal was simple: to look like a refreshed version of her younger self.

She’s now about 3 months post-op following:
• Deep Plane Facelift
• Fat Grafting
• Lip Lift
• Mini Rhinoplasty
• Blepharoplasty

It’s been beautiful to watch everything settle in so naturally. Stay tuned for the rest of her journey.

1.5 months post-op (video)

This patient presented with concerns related to facial and neck aging, including loss of jawline definition, neck fullness/laxity, and changes around the eyes. A comprehensive surgical approach was used to address the deeper structural changes while maintaining a natural appearance.

Her treatment plan included:
Deep neck contouring
Deep plane facelift
Brow lift
Blepharoplasty
Fat grafting
DEKA CO₂ laser to the face and neck

The neck will define facelift surgery in 2026 and beyond.

Beautiful results aren’t created by tightening skin alone. They come from carefully addressing the deeper layers of the neck — where contour and support actually live.

When the foundation is refined, the result looks natural, balanced, and long-lasting.

Every surgeon starts somewhere. Larian and I have been friends since our mid-20s. I’m grateful that all these years later, we’re still working side by side.

22 years ago, I performed Vicki's first facelift.

She was 49 at the time. Two decades later, at 69, she returned for a secondary facelift. Today she is 70 and looks absolutely stunning 1.5 years post-op.

One of the greatest privileges of my work is building lifelong relationships with my patients—and Vicky is one of them.

Her treatment plan included:

• Browlift
• Deep plane face and neck lift
• Lip lift
• Fat grafting
• CO₂ laser to the face, neck, and chest

Patients like Vicky are a powerful example of how early surgical intervention and thoughtful maintenance can help preserve natural beauty as we age.

Regenerative medicine gets talked about a lot in the aesthetics space and for good reason. Treatments like exosomes, microneedling, PRP, and biostimulators are designed to support the skin’s natural repair process and improve texture, tone, and glow gradually.

Subtlety is the new luxury.

In a recent conversation with Bloomberg, I discussed the growing demand for discreet rhinoplasty — procedures that refine without announcing themselves.

Many patients today aren’t seeking a different nose.
They’re seeking balance, harmony, and results that feel natural and age-appropriate.

The goal isn’t transformation.
It’s refinement.

Thank you to Alina Dizik for the thoughtful coverage.

Age 50 ➝ Age 53
2 years, 9 months follow-up

There’s something incredibly special about seeing someone not only look refreshed but look like herself again. This transformation was never about changing her features. It was about restoring softness, balance, and confidence in a way that still feels completely natural.

Her comprehensive treatment plan included:

• Upper blepharoplasty
• Face & neck lift
• Fat grafting
• Selective neurolysis
• Fractional laser (Sylfirm)
• Consistent Botox maintenance

Nearly three years later, her results remain beautiful and understated—natural, balanced, and full of expression.
