🦵 My patient is halfway through her circumferential thigh liposuction surgery and we’ve already removed 5 liters of fat.

After her last surgery, her calves and ankles look amazing, with reduced cuffing and improved definition. Now we’re tackling the knees, and releasing a lot of stubborn fibrosis.

Many surgeons hesitate to go near the knee cap, but it’s crucial because fibrosis often builds up at the knees and elbows. Already, her knees are starting to take beautiful shape, and we can’t wait to see the final transformation!

When comparing areas of fat in Lipedema, you can actually see a difference in coloring and texture.

⚪ Fat that appears more white is typically more fibrotic. It feels dense and firm, almost like cement.

🟡 Fat that looks more yellow, like chicken fat, is less fibrotic and has a softer consistency.

These differences matter, because the degree of fibrosis can impact symptoms, pain, and the surgical approach.

🔎 What is the pelvic girdle?

When we talk about lipedema surgery, it’s important to know where lipedema is and isn’t. The shadowing in the buttock area defines the pelvic girdle, and extends up toward the small of the back, and wraps around to the front near the inguinal area.

For this patient, we’ll be addressing the thigh unit in a separate surgery, and decongest proximally first, starting with the pelvic girdle and lower legs.

I’m outside of NBC Studios in New York, where I was interviewed by from the about a growing threat within the lipedema community.

A company called Svelta Venastra is using AI-generated videos to promote a fake “miracle cream,” stealing the identities of myself, Dr. Karen Herbst, and even public figures like Oprah, Kelly Clarkson, and Carnie Wilson.

What concerns me most is that these scams prey on vulnerable patients who are doing everything they can to manage their condition. I care deeply about protecting lipedema patients from being misled or taken advantage of and I’ll continue using my voice to expose those who exploit their trust.

I’m grateful to the Today Show for bringing national attention to this critical issue. Tune in on October 28th at 8 AM EST.

💡 Lipedema in men can often be overlooked.

This is because the condition is not often typically expressed in men, unless there’s a hormonal imbalance. Signs can be subtle, like a feeling of fullness in the legs, increased swelling, and digestive issues such as gluten intolerance, bloating, or leaky gut syndrome.

As it progresses, fibrosis can also start to develop just as it does in women. Today, my male patient came in for treatment, proving that lipedema can look different than what you expect

She was treated for Stage 2 Lipedema and is now over a year out from Lipedema Reduction Surgery with myself and a brachioplasty with The Roxbury Institute’s Head of Plastic Surgery.

Before surgery, pain and mobility limitations held her back from fully engaging in the activities she loves. Now, the difference is truly astounding, and we are so proud of her courage, dedication, and the joy she’s reclaiming in her daily life.

📝 I was recently featured in Medscape Medical News in an article titled “Weight of Two Worlds: Lipo-Lymphedema and Obesity,” which highlights the story of my patient, Shannon Ashley.

For years, Shannon was dismissed by doctors who told her she was “just fat.” Unfortunately, this is something I hear far too often from patients living with Lipedema and Lipo-Lymphedema.

These conditions are still under-recognized, and that lack of awareness leads to misdiagnosis, stigma, and delayed care. This is why education and advocacy are so important.
