External root resorption: Extensive root resorption detected on periapical radiograph and then confirmed with CBCT scan. The resorption had destroyed the root and it could not be restored. The teeth were splinted together with composite, and the the area was exposed and the root was sectioned and removed. The defect was grafted with cortico-cancellous allograft and the flap was sutured back with 7-0 PGA sutures. ⁠⁠

Lateral sinus lift. There are different approaches for gaining bone in the maxillary sinus. My criteria is if there is less than 4 mm of residual bone and a very large sinus, then I prefer the lateral approach with delayed implant placement. This particular patient is in his late 20’s and I wanted to provide him with an option that will afford him the best prognosis for a life time restoration. This is a very traditional but tried and true way of completing a sinus lift. ⁠
PRF membrane was placed against the sinus membrane to improve healing and the sinus was filled with mineralized cortico cancellous allograft mixed with liquid PRF. We will allow this bone to mature for 4-5 months and then place the implant. ⁠

Deep pockets and recession were present on the upper central teeth. Clinically I could feel roughness on the roots. A conservative flap was elevated to expose the root surfaces and allow access to root preparation. As you can see there was heavy calculus and root roughness present, which would have been impossible to manage with scaling and root planing alone. Once the roots were prepared the smear layer was removed with Pref Gel (EDTA) and Emdogain was applied to promote new attachment to eliminate the deep pockets. The tissue was then coronally advanced to address the gingival recession and sutured with 6-0 Glycolon from Resorba. us

This patient was referred to me to figure out why there was persistent bleeding and pocketing even though the area had been deep cleaned and Arestin (antibiotics) had been placed on a few occasions. Tooth is vital (no root canal). A flap was elevated which revealed heavy calculus deposits. The caucus was removed with root planing, Straumann Pref Gel was used to remove the smear layer and Straumann Emdogain was applied and the flap was sutures back. ⁠⠀⁠

Post op pain level was 0-1 and healing was rapid. I saw the patient this week for 2 year follow up and there is no pocketing and bleeding. ⁠⠀⁠

Take home lesson is Arestin does not work if the calculus is left behind. ⁠⠀⁠

To treat the recession on multiple adjacent teeth on this patient, the approaches I thought of were using CTG and then either Zucchelli, or tunnel. Initially I thought of a lateral pedicle to cover the canine, but did not want to cause an esthetic issue for the lateral incisor. So I ended up tunneling from the molar to the lateral incisor and was able to get excellent coronal advancement, but ended up reflecting the papilla between the canine and the first premolar to eliminate the excess tissue bunching and then used a thick CTG and coronally advanced the tissue over it. The final photo is 20 days post op.⁠⠀⁠

Patient wanted a smile makeover due to discolored teeth and restorations, and gingival asymmetry. After meticulous work up, gingival grafting was completed (palatal connective tissue and CAF) to cover the exposed roots at 9, 10, 11, 12, and allowed to mature for 2 months. The teeth were restored with combination of porcelain veneers and full coverage crowns. The patient wanted to have natural appearing restorations and did not want to have them be too white. Without gum grafting we would not have been able to accomplish an esthetic outcome. ⁠⠀⁠

We tell a patient they have an infected tooth that should be removed. Some patients resist because they have no pain and they don’t see the need or urgency to have treatment on a tooth that is not hurting. ⁠⠀⁠

This case illustrates one such case. This infected tooth was not causing pain, but when the infected tissue that surrounded this tooth is stretched across the back of my hand we can we see the large size of such a wound. I don’t believe any patient would ignore a bleeding and infected wound like that if it were on the back of their hand or their face. ⁠⠀⁠

1. Pain usually occurs when there is a fast moving infection. ⁠⠀⁠
⁠⠀⁠
2. Slow or chronic infections usually do not have pain as one of their symptoms.⁠⠀⁠
⁠⠀⁠
3. Chronic infections can and do contribute to whole body inflammation. ⁠⠀⁠
⁠⠀⁠
4. Inflammation is thought to be the main culprit of many other diseases and is a major contributor to aging. ⁠⠀⁠

Implants, like natural teeth, are prone to “gum disease.” Plaque and bacterial buildup can form calculus (tartar) on the implant surface which is difficult to clean. That is why it is extremely important to have regular dental cleanings (every 3-6 months) if you have dental implants. ⁠⠀⁠
