Perimenopause brings significant changes to the skin, primarily driven by declining estrogen levels.

Here’s what many women experience during this transition:

Dryness and moisture loss: This becomes increasingly noticeable as estrogen levels drop. Estrogen helps maintain skin hydration by supporting the production of natural oils and hyaluronic acid, so when these hormones decline, skin often feels tighter, rougher, and more prone to flaking.

Loss of elasticity and firmness: This occurs because estrogen stimulates collagen and elastin production.

During perimenopause, collagen production can decline by up to 30% in the first few years after menopause begins. This leads to sagging, particularly around the jawline, neck, and under the eyes, along with more pronounced wrinkles and fine lines.

Thinning skin: Skin becomes more fragile and translucent as the dermal layer loses volume. This makes blood vessels more visible and increases susceptibility to bruising and injury.

Changes in texture and tone are also common, with many women noticing increased dullness, uneven pigmentation, and age spots (hyperpigmentation). The skin’s natural cell turnover slows down, leading to a less radiant complexion.

I’ve been receiving quite a few DM’s recently regarding supplements I take for menopause. My daily menopause stack features:

➡️ Oral progesterone ( I take it every night )
➡️ Estradiol and testosterone cream combined

After running my own labs, I discovered I was low on progesterone and a few other markers were within range, but definitely not optimized.

During menopause this is extremely common. Since the body produces less estrogen and progesterone, leading to symptoms like hot flashes, night sweats, mood changes, and vaginal dryness.

I’ve found that taking these hormones together has helped relieve my symptoms.

The other day I met with a patient who was experiencing every menopausal symptom you could think of. Hot flashes, weight gain, irritability, insomnia…

After reviewing her labs, it was evident she was experiencing some of the classic symptoms of low estrogen.

She immediately expressed that she was apprehensive about starting it (even at a low dose) because she heard “HRT causes cancer.”

I know she is one of many who are missing out on the benefits of HRT for this reason. So, I wanted to take a moment and share some info about HRT with you all.

Multiple large studies, including the Women’s Health Initiative, have consistently shown that estrogen-based HRT actually reduces the risk of colorectal cancer by approximately 30-40%.

The protective mechanism appears to involve estrogen’s effects on bile acid metabolism, reduction of insulin-like growth factor levels, and anti-inflammatory properties in the colon.

Studies also show that estrogen-based bioidentical hormones can also prevent breast cancer.

Studies show the cancer-preventive effects of HRT work through several biological pathways:

Hormonal balance: Restoring declining hormone levels may prevent the cellular changes that occur with dramatic hormonal fluctuations.

Anti-inflammatory effects: Estrogen has anti-inflammatory properties that may reduce chronic inflammation, a known cancer risk factor.

Metabolic regulation: HRT can improve insulin sensitivity and metabolic markers associated with cancer risk.

Muscle becomes crucial during perimenopause for a number of reasons…

For starters, estrogen levels drop DRAMATICALLY, and estrogen actually helps maintain muscle mass.

Without it, women can lose muscle pretty quickly - we’re talking about potentially losing 3-8% of muscle mass per year during the perimenopausal transition.

Less muscle means your metabolism slows down, so it becomes easier to gain weight and harder to lose it.

Muscle tissue is metabolically active - it burns calories even when you’re just sitting around. So maintaining muscle can help with weight management (especially when fat tends to accumulate around the midsection).

Your bones also get weaker during menopause, and having good muscle mass helps protect against fractures, falls, and osteoporosis.

The good news is, strength training, eating enough protein, and balancing your hormones/making sure you have enough estrogen can really help counteract this muscle loss.

Estrogen is far more than just a “female hormone”. It’s a powerful chemical messenger that orchestrates hundreds of biological processes throughout the body.
Estrogen’s influence extends to virtually every organ system, certainly not just reproductive health, making it one of the body’s most versatile and essential hormones.

Low estrogen is associated with saggy skin and wrinkles, weight gain, low libido, mood swings, insomnia, and so many other symptoms.

As a physician who works with many patients on GLP-1’s and other peptides, I’m continually amazed by the positive changes I see in my patients that extend far beyond just the number on the scale.

While weight loss often brings patients interested in GLP-1’s to my office initially, I love seeing the profound ripple effect these peptides create across multiple aspects of health.

For example, patients who have struggled with high blood pressure for years often experience significant reductions after starting GLP-1’s,

The sleep improvements are consistent and significant as well. Patients report deeper, more restorative sleep, which then creates positive feedback loops with mood, energy, and decision-making. Many find their chronic joint pain diminishing not just from weight reduction, but seemingly from the anti-inflammatory effects of the medication itself.

I’m also seeing improvements in conditions I wouldn’t have initially connected like reduced cortisol levels and improved mood/less PMS symptoms.

It’s time we break the silence around perimenopause and menopause. 🗣️

For too long, women have been expected to quietly endure the profound physical and emotional changes that come with this life transition.

We’re told “it’s a part of aging.”
“It’s something we just have to ride out.”
“It’s just a part of life and womanhood.”

The dismissal of symptoms… the lack of open conversation…and the absence of proper validation leaves so many women feeling isolated and confused about what’s happening to their bodies and minds. They’re told it’s normal to feel crazy and like we are losing our minds.

Here’s the thing though…
These aren’t just “minor inconveniences” to push through, they’re significant life changes that deserve acknowledgment, understanding, and support.

Most women might not know this, but there’s actually a non-synthetic form of hormone replacement therapy…

Bioidentical hormones.

Bioidentical hormones are basically hormones that are chemically identical to what our body naturally produces. 

Synthetic hormones (standard HRT) are chemically different versions that your body processes differently.

The main difference between the two comes down to molecular structure. 

Bioidentical hormones, whether it’s estrogen, progesterone, or testosterone, have the exact same chemical makeup as the hormones our ovaries, te**es, or adrenal glands would make. 

Synthetic hormones, on the other hand, are altered versions that pharmaceutical companies developed partly so they could patent them (you can’t patent something that occurs naturally in the body).

Many people report fewer side effects with bioidentical versions because your body recognizes and processes them more naturally.
