There are a few things worth knowing about why Enhanced Internal Medicine exists.

Dr. Paluga trained at the University of Michigan, taught at Northwestern Medicine and Michigan Medicine, and came to Birmingham to build something that the conventional care model rarely allows: a practice where she could give every patient the time, the access, and the continuity that actually changes health outcomes.

The membership model is not an upgrade on conventional care. It is a different structure entirely. Fewer patients. More time. A physician who knows you well enough to notice what has shifted before you have found the words to describe it.

That is what this practice was built around.

Prevention is always the more powerful conversation. Skin Cancer Awareness Month is a good reminder of that.

Skin cancer is the most commonly diagnosed cancer in the United States, with over five million cases treated each year. What makes that number worth sitting with is this: the majority of those cases were preventable. Not with something complicated. With consistent daily sun protection and annual skin checks.

SPF 30 or higher, broad spectrum. Applied every morning, not just on sunny days, not just in summer. Reapplied every two hours when you are outdoors. And if something on your skin has changed in size, shape, or color, that is a conversation worth having sooner rather than later.

The best time to talk about prevention is before there is a reason to be concerned. That is exactly the kind of conversation that belongs in a comprehensive annual exam.

There is a particular kind of anxiety that comes with waiting for lab results.

The notification that takes days. The number on a portal that means nothing without context. The follow-up call you are not sure will come. Most people have learned to manage that gap on their own because the conventional care model was not built to close it.

At Enhanced it does not exist.
Your labs are drawn in the office and reviewed with Dr. Paluga during the same visit. In-office EKG is available and interpreted the same day. You leave with your results, with context, and with a physician who has already walked you through what they mean for your health and what comes next.

That is not a luxury. It is simply what an appointment looks like when it was designed to actually be complete.

Most people do not need a study to tell them their mental and physical health are connected. They feel it.

In the sleep that never quite restores. In the tension that has become so constant it barely registers. In the way a hard season at work eventually shows up in the body as something that needs a name.
May is Mental Health Awareness Month. For Dr. Paluga, it is a reminder of something she thinks about in every single appointment, because she has never believed that asking how you are doing is a box to check at the end of a visit.
It is where the visit begins.

How you are sleeping. How you are managing what life is asking of you right now. Whether the low-grade anxiety that arrived six months ago has a pattern she should understand. Whether the exhaustion you described is emotional, physical, or, as it so often is, both at once. These are not add-ons to your care. They are the foundation of it.

The science has been clear for decades. Chronic stress drives cardiovascular disease. Depression alters inflammatory markers. Sleep disruption destabilizes hormones and immune function. The line between mental and physical health is not a line at all. And yet the conventional care model continues to treat them as though they belong in separate rooms, separate referrals, separate conversations, with no one ever holding the full picture.

Dr. Paluga holds the full picture. That is not a feature of this practice. It is the philosophy behind everything we do.

If your physician has never asked how you are actually doing, not the checkbox version, but the real question, that is worth noticing.

Women's health and mental health are not separate categories.
They are the same story. The hormonal shifts that mark every stage of a woman's life are not purely gynecological events. They are neurological ones. They modulate mood, sleep, stress response, and cognitive function. When they fluctuate, everything else does.

And yet the conventional care model continues to treat them as though they have nothing to do with one another.

This month, we wrote about what that costs women and what becomes possible when one physician finally holds the whole picture.

Most women have a version of this story. You describe a symptom, exhaustion that sleep does not fix, anxiety that arrived without an obvious trigger, and cycles that have become something you dread rather than simply manage.

Finding the right physician is not a small decision and we do not treat it like one.

Before anyone joins Enhanced Internal Medicine, we invite them to sit down with Dr. Paluga for a complimentary 30-minute meet-and-greet. No commitment, no obligation, no sales pitch. Just a real conversation about your health history, what you have been looking for in a care relationship, and what membership at Enhanced actually looks like day to day.

We want to understand what you need. You should leave knowing exactly what we offer and how we practice. If Enhanced is the right fit, that will be clear. And if it is not, we would rather know that too.

What we are building here is a proactive partnership we hope lasts years, if not decades. It should start with the right conversation.

Schedule your complimentary meet-and-greet at enhancedinternalmed.com or call us at (248) 609-1440.

Most mothers are experts at taking care of everyone except themselves.

Not because their health does not matter to them. Because the system was not built to hold everything they bring to it. The fatigue that never fully lifts. The stress that has become so constant it barely registers anymore. The hormonal changes that get attributed to a busy life rather than investigated as the clinical matter they are.

Research shows that 65% of working parents experience burnout. Mothers carry nearly three quarters of all cognitive household labor, the invisible mental load of managing a family, a career, and everything in between. And when they do make it to a doctor's appointment, the average visit lasts 15 minutes. That is not enough time.

At Enhanced, Dr. Paluga has the time to ask about all of it. The sleep, the stress, the hormonal shifts, the things that exist just beneath the surface of what usually makes it onto a chart. She understands from personal experience what it means to hold that kind of fullness, as a physician, a business owner, and a mother of three.

Mothers deserve that level of care. We are here to offer it.

The number on your scale has never told the full story. The InBody scan does.

This is a clinical body composition assessment used by leading hospitals, research institutions, and medical practices across the country. At Enhanced Internal Medicine, it is part of how Dr. Paluga builds a complete, data-driven picture of each patient's health from day one.

Here is what a single InBody scan reveals:
Skeletal muscle mass, measured by segment so you can see exactly where your body is strong and where it is losing ground. Body fat percentage and distribution, including visceral fat levels, which carry far greater cardiovascular and metabolic risk than subcutaneous fat and are invisible on a standard scale. Total body water and cellular hydration, markers that reflect inflammatory status and overall cellular health. Basal metabolic rate, your body's caloric baseline at rest. And a segmental analysis that shows the balance, or imbalance, across your arms, legs, and trunk.

For patients using GLP-1 medications like tirzepatide or semglutide, this assessment is not optional, it is essential. One of the most underreported risks of GLP-1 treatment is lean muscle loss. The scale may show a lower number while significant muscle is quietly disappearing, which has real consequences for metabolism, strength, bone density, and long-term health outcomes. Regular InBody monitoring allows Dr. Paluga to track exactly what is changing and adjust the care plan accordingly.

Join us today for our ribbon cutting at 122 E. Brown Street in downtown Birmingham.

Complimentary InBody scans are available for every guest. It takes minutes and leaves you with information most annual exams never provide.
