Some stories end sooner than we ever feel ready for.
But for the people left behind, the story continues. 🤍

At Live Well Hospice, we believe hospice care is about more than end-of-life support. It’s about walking beside patients and families through every stage of the journey, and for families, that journey continues after the passing of the loved one.

Our support doesn’t stop after a loved one passes. We follow families for 13 months after loss, helping them navigate grief, the “firsts,” and the difficult moments that follow with ongoing bereavement support and compassionate guidance.

Because grief doesn’t follow a timeline, and because life, does in fact, carry on. But we don’t believe you should feel you’re carrying it alone.

Choosing hospice is never an easy decision, especially when it comes after years of fighting, treatments, and hoping for more time together.

Her mother made the deeply personal decision to stop dialysis, and choose comfort-focused hospice care instead. Even though she was only under Live Well’s care for a short time, our team made sure both she and her family felt supported, cared for, and never alone through those final days.

Because hospice care is about more than medical care.
It’s about compassion.
It’s about dignity.
It’s about checking on family members after a loss.
It’s about handwritten cards filled with signatures from people who truly cared.

At Live Well Hospice, we believe support shouldn’t end the moment a loved one passes. That’s why we continue walking alongside families with bereavement support, guidance, and follow-up care during the difficult “firsts” that come after loss.
