As you may have heard, our office will be closing permanently on October 31st, a couple weeks before the original date we had posted, which will also be the last day to pick up medical records.
To receive your records, you may complete a medical release form either in our office or online. If you already have a new provider, we can send your records directly to their office electronically.
Our staff is working diligently to prepare and print all records, so please be patient with us as we complete this process.
We are currently experiencing a very high call volume and expect this to continue over the coming weeks. We are doing our best to return every call, but it may take a few days before we can respond.
