***some at-home COVID-19 tests have had the expiration dates extended. Please see the FDA website to determine if the test you have has has the date extended and what the new date is.

We are having technical difficulties with our phone lines. If anyone has been trying to call us we are not receiving any calls or voicemails at this time. We have reached out to technical support and hope to have the issue corrected ASAP! In the meantime, please utilize the patient portal for appointment requests/medication refills/ or questions. We apologize for any inconvenience!

Due to repetitive no shows, last minute cancellations, and late arrivals we will have the following policy updates effective June 3, 2024. We value our patients time and make an effort to be able to offer appointments with limited wait times. When patients are late for their appointments this can create longer wait times for others. When patients do not show this is a spot that could have been available for someone that needed it. We hope that you will understand these changes and we look forward to the opportunity to service your health care needs.

Late policy- Appointments will automatically be cancelled and labeled as a no show after 5 minutes.

No show: There will be a $25 fee for all no show appointments. This will also apply to appointments made for the same day. Appointments scheduled same day and cancelled or result in a no show will also be subject to a $25 fee. Repetitive no shows and last minute cancellations/reschedules may result in a fee or dismissal from the practice.
