ONE FALL CAN CHANGE IT ALL- LET’S PREVENT IT TOGETHER

September is fall prevention month, and Mountain Home Health is here to help patients stay steady and safe at home!
Did you know that falls are the leading cause of injury for older patients, but with the right resources, they are often preventable. Let’s work together to lower injury- related hospitalizations, hip fractures, and loss of independence!

Common Fall Risk Factors
• Unsafe home environment
• Poor balance or muscle weakness
• Medication side effects (dizziness, drowsiness)
• Chronic conditions such as arthritis, diabetes, or stroke
• History of previous falls

How can our home healthcare team help prevent falls?
• Comprehensive Fall Risk Assessment- our team will assess the home and health condition to identify any fall risks.
• Home Safety Recommendations- we help reduce risks by suggesting grab bars and handrails and also removing trip hazards.
