Job description:

The medical social worker is responsible for the implementation of standards of care for medical social work services.
Essential Job Functions/Responsibilities
1. Assesses the psychosocial status of patients related to the patient’s illness and environment and communicates findings to the registered nurse.
2. Carries out social evaluations and plans intervention based on evaluation findings.
3. Maintains clinical records on all patients referred to social work.
4. Provides information and referral services for Organization patients and families/caregivers regarding practical and environmental needs.
5. Provides information to patients or families/caregivers and community agencies.
6. Serves as liaison between patients or families/caregivers and community agencies.
7. Maintains collaborative relationships with Organization personnel to support patient care.
8. Maintains and develops contracts with public and private agencies as resources for patient and organization personnel.
9. Participates in in-service education programs.
10. Participates in the development of the total plan of care and case conferences as required.
11. Assists physician and other team members in understanding significant social and emotional factors related to health problems.
12. Participates in discharge planning.
13. Assist patients with In-Home Supportive Services (IHSS) Program.
14. Other duties as delegated by the Clinical Director/Supervisor.

Job Title/Position: Medical Social Worker
Position Qualifications
1. Graduate of a Master’s program in social work accredited by the Council on Social
Work Education.
2. Licensed as a licensed clinical social worker by the Board of Behavioral Sciences.
3. Minimum of one year's experience in health care.
4. Experience in a home health care preferred.
5. Demonstrates good verbal and written communication, and organization skills.
6. Possesses and maintains current CPR certification.
7. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

🖐️We need volunteers ‼️ We are seeking meal delivery volunteers in several areas, with openings each day of the week. We deliver meals Monday through Friday between 10am-12pm. The routes take between 30 minutes to 2 hours to complete. Volunteers use their own vehicle, must be over the age of 18 (or accompanied by a parent/guardian), and have a valid drivers license and vehicle insurance.

The Friends of the Valley Community Library’s fall book sale will take place on Friday, November 15 from 10:00 a.m. to 4:30 p.m. and Saturday, November 16 from 10:00 a.m. to 2:00 p.m. Patrons may fill a provided bag on Saturday from 12:00 p.m. to 2:00 p.m. for $5.

There will be a special presale for Friends of the Library members only on Thursday, November 14, from 4:00 p.m. to 6:30 p.m. Purchase a membership at the door for $10 individual/$15 families. The book sale will be held inside the library.
