Primary Care of Southwest GA, Inc was established in 2005 and opened a Federally Qualified Community Health Center for business in Blakely, Georgia, in March 2006. In August 2007, a second site was opened in Thomasville and was approved as a Federally Qualified Health Center in November 2011. In March 2015, we opened our Women's Health Center (formerly known as Midwifery Center) in Thomasville and opened a 4th Primary Care site in Bainbridge in August 2015 and the 5th location in Quitman April 2017. In January 2018, PCSG opened its first school based health center at Thomas County Middle School. PCSG has now been fortunate to open its second school based health center in Blakely, Georgia, at the Early County Elementary School. PCSG is a non-profit corporation which is governed by a local board of directors made up of community members, 51% of whom are required to be active patients. PCSG serves the broad health needs of Southwest Georgia residents through a family practice model of care. PCSG works to ensure that its patients have access to all levels of the health care system, and is especially committed to providing services for the medically underserved. PCSG is a strong advocate of preventive education and public health activities. Primary Care of Southwest Georgia collaborates with other community agencies and organizations, i.e. Local Hospitals, Spring Creek Health Cooperative, and Public Health, to identify health related needs and to develop resources to facilitate these needs.
