Columbus is pleased to announce our partnership with PocketRN and our participation in the Medicare Guiding an Improved Dementia Experience (GUIDE) model as a GUIDE Partner. Through this initiative, we are working to expand access to dementia care for individuals living with Alzheimer’s and dementia, as well as their caregivers. As part of CMS' eight-year pilot program, Columbus is contributing to efforts aimed at improving support and resources for those with a diagnosis of Alzheimer’s or dementia.

This partnership highlights our ongoing commitment to pursuing collaborations and innovative programs that advance care for the dementia community. We remain dedicated to empowering families with meaningful resources and compassionate support. 💜

As part of our ongoing efforts to develop the most advanced capabilities to reduce risk and improve care for the clients we serve, we’re excited to share that Columbus has partnered with the AlzBetter Dementia 360 Program to strengthen support for individuals living with dementia and the caregivers who support them. Through this initiative, our care managers will receive specialized training designed to enhance their ability to provide effective, person-centered dementia support focused the caregivers of individuals living with this diagnosis.

The goal is simple but powerful: improve quality of life, reduce caregiver strain, and help individuals remain safely in their homes and communities for as long as possible. 💜

The AlzBetter app will play a key role by offering caregiver education, ongoing support, and a robust library of lessons and resources - including practical guidance for navigating common dementia related challenges. This partnership reflects our ongoing commitment to delivering high quality, innovative, and compassionate services for those we serve.
