This patient underwent a rhinoplasty many years ago but was unhappy with the result and did not wish to have revision surgery. Revision rhinoplasty often requires cartilage grafts from the rib, ear, or a cadaver. Instead, we used strategically placed hyaluronic acid filler to improve tip projection and smooth the nasal bridge and other irregularities.

Using filler after a rhinoplasty carries increased risks, regardless of how much time has passed—even if it’s been 1.5 or 30 years. These risks stem from scar tissue and vascular changes. Scar tissue can raise pressure in the area, increasing the likelihood of vascular occlusion, which occurs when filler blocks or compresses a blood vessel. This impairs blood flow, limiting oxygen delivery to the tissue. If not recognized and treated promptly, this can lead to tissue necrosis.

It is crucial to see an experienced injector—nose filler is not something to take lightly.
