As I saw patients in my clinic during residency training to become a neurologist, I felt frustrated. The waiting room was filled with patients, some of whom had been waiting for hours. I had 12 patients on my schedule for the afternoon, what was supposed to be a four hour clinic, and they were all new to me. Most did not speak English. Most had complex medical conditions. Some had memory or language problems. Some came in wheelchairs from their nursing home, accompanied only by an aide who was totally unfamiliar with them. If you do the math, I had only 20 scheduled minutes on average with each patient. In that time, I had to interview the patient and their family members (often with the use of a translator), perform a complete examination, present it to my attending physician, who then would go and see and examine the patient themselves, write prescriptions, send referrals, and then write my note. If youāre thinking itās impossible to do that and still do a good job, it is. In fact, itās impossible, period. Most of my colleagues waited until after clinic to write all their notes, and they found their āfreeā time was rapidly becoming consumed with administrative duties. The experience was soul-crushing. Thereās no wonder thereās a āburnoutā epidemic among physicians.
Patients would come in angry. Why have I had to wait for so long? they would ask. Why donāt you speak my language? Why are you rushing me? I want to tell my whole story.
If you or a family member have ever been on the āpatientā side of medicine, you know how frustrating and demoralizing it can be. You lay your life on the line with your doctor, only to be interrupted or just not listened to. You get sent to test after test. No explanation. No chance to ask your doctor questions. Getting prescribed new medications, hoping for the best. Then, you think of some questions later. You call your doctorās office, only to reach an answering service or a medical secretary. You leave a message, but who knows when the doctor will have time to call you back if at all?
Frustrating on both sides. Lose/lose.
