At First Choice Home Health & Hospice our care is tailored to the needs of each individual and is delivered on a personalized level with innovative health care techniques. There are such positive feelings that all of us associate with being home. Home Health Care is skilled nursing care, physical therapy, speech therapy & occupational therapy provided directly to you in your home. Medical Social Work is another service which may be provided and certain other health care services that you may receive in your home for the treatment of an illness, surgical rehabilitation, or injury. At First Choice, our Hospice Team offers comfort and care to patients & their loved ones as they enter one of the most difficult times of their lives. Our hospice services are available to patients, caregivers, & family members, assisting them through the transition from curative care to comfort care. We also offer bereavement support to all who seek our help. We provide comfort care either in our patient’s home or in a comfort care facility, depending upon the needs of the patient. In order to ensure the best possible care, our team is composed of licensed and certified professionals who are passionate about hospice. We do all we can to ease the emotional burdens on the family, while honoring the patient’s final wishes.
