At Auburn Crest Hospice, we believe in choosing to live every moment. 🦋

Today, our hospice social worker spent the afternoon at one of the facilities where we care for patients, making ice cream sundaes with the residents. The room was filled with laughter, smiles, and sweet memories. One resident said, “This is a good day,” while another enjoyed their treat and declared it “scrumptious!” 🍨✨

The care staff loved getting to join in too—especially when it involved yummy treats! Moments like these bring everyone together, reminding us that hospice is not only about care but about joy, comfort, and connection.

This is hospice. Choosing to live every moment. 🦋

At Auburn Crest Hospice, we love finding ways to serve not only our patients, but also our amazing community partners. 🦋

This week, our team gathered for a Junk Journal Service Project Party 🥳 to create one-of-a-kind, handmade journals for the residents of Ashley Manor Cory Lane Assisted Living. They reached out asking for junk journals as Christmas gifts for their residents, and we couldn’t wait to get to work!

These journals will be filled with personality, textures, and little surprises — perfect for sparking memories and inspiring creativity. It’s such a joy to pour love and care into something that will brighten the holidays for someone else.

We’re so grateful to work alongside partners like Ashley Manor, and to have a team that loves giving back in meaningful ways. 💙

At Auburn Crest Hospice, we believe in our motto — “Choose to live every moment.”
One of the ways we help our patients do just that is by making sure their birthdays are celebrated in a special way.

Today, we had the joy of celebrating Patricia’s 95th birthday! 🎉 Thanks to our amazing volunteers, she enjoyed a beautiful homemade cake 🎂 made just for her. These moments bring smiles, laughter, and sometimes even tears of joy — a beautiful reminder that life is meant to be cherished, no matter the season we’re in.

We’re so grateful for our volunteers who pour their hearts into baking and decorating these cakes, and for the privilege of helping our patients keep living, loving, and making memories. ❤️ 🦋

At Auburn Crest Hospice, we love changing up the scenery for our team meetings — and today was something special.

We gathered at the beautiful Old Valley Nursery, owned by our amazing chaplain Greg, and spent the morning surrounded by blooming flowers, thriving shrubs, and peaceful trees 🌸🌿🌳. With coffee, donuts, and good conversation, we took a fun personality test and learned more about each other — and about plants, thanks to Greg’s expertise!

One of the highlights? We captured a video and photo of a hummingbird zipping through the nursery— a sweet reminder to slow down and enjoy life’s little wonders. 🐦

We wrapped up with some great staff photos, full hearts, and a renewed sense of connection. Huge thanks to Greg for hosting us in such a peaceful and inspiring space. 🦋

🎄☀️ Christmas in July at Auburn Crest Hospice!

This month, we brought some unexpected holiday cheer to the facilities where we have hospice patients —because who says Christmas magic is just for December? 🎅🦋

Our team showed up in full festive spirit with caroling and a special visit from Santa in vacation mode! He traded his sleigh for sandals and surprised residents with thoughtful Christmas gifts for everyone. 🎁🎶

The smiles, laughter, and warm memories made this a day to remember. We are so honored to walk alongside these wonderful souls and their caregivers, not just in care—but in celebration.

She gasps like she just saw Beyoncé, whips out a bag, and starts carefully harvesting fresh buds like she’s in a Jane Austen novel.

“Some of our new patients asked for lavender baths,” she says, stuffing her purse with fragrant sprigs like a Mission: Impossible pro.

So if anyone’s looking for the person who “gently borrowed” some hospital landscaping — she did it for hospice love. 💜😅

Every 14 days, our team comes together with our Medical Director, Dr. Moulton, for Interdisciplinary Team (IDT) meetings to discuss every single patient in our care.

This isn’t just a regulatory requirement — it’s our commitment to ensuring each person receives a personalized, compassionate, and coordinated plan of care.

Together, our nurses, social workers, chaplains, aides, and leadership:
✅ Review patient updates
✅ Adjust care plans as needed
✅ Address medical, emotional, and spiritual needs
✅ Support families every step of the way

When the boss is away… the team will have their bi-weekly IDT wherever they want! 🤪

Why settle for a conference room when you can discuss care plans with birds 🐦 chirping, lavender lemonade 🍋 flowing, butterflies 🦋 fluttering and someone’s rottsky 🐶 judging your pain management notes? 🤣

This week’s Interdisciplinary Team meeting took place in our marketer’s backyard—because what says collaborative patient care like lawn chairs, sunscreen, and hot dogs 🌭 and hamburgers 🍔 grilling?

At Auburn Crest, we are proud to celebrate our incredible Certified Nursing Assistants (CNAs)—the true backbone of hospice care. This week, we honor the compassion, dedication, and tireless service of the CNAs who walk alongside our patients and families every day.

In hospice, the role of a CNA goes far beyond physical care. Our CNAs provide comfort, dignity, and emotional support during some of life’s most vulnerable moments. They are the calming presence, the gentle touch, and often the smiling face our patients rely on most. Whether assisting with daily tasks, offering a listening ear, or simply holding a hand—they bring humanity and heart to every visit.

At Auburn Crest, we are deeply grateful for the work our CNAs do. Your kindness, professionalism, and commitment do not go unnoticed. Thank you for being the heartbeat of our team.
