Hello to our favorite Community,

We are reaching out with sincere gratitude and an important update.

As you know, South Mountain Community Health has always believed in deep community connection and we are ever committed to our patients and to this community that gave us a home 22 years ago. Your health, your stories, and your trust are why we do what we do. You are an equal part of this circle we have built and grown to treasure. It is a daily honor to welcome all of your family generations and it is this support that reinforces our "way" of care.

Earlier this year, we explored a potential partnership with another healthcare organization with the shared goal of expanding resources and improving access to care. After thoughtful and mutual consideration, it was decided that South Mountain Community Health will continue to operate independently as the community-centered, patient-focused practice you have always
known.

This exploration process gave us a chance to look at our current (2 decades old) systems in electronic medical records keeping, IT, phone systems and work flow. All of these and more have been recently enhanced and over the next few weeks, the support staff and providers will be trying out their new knowledge as they work with you from the beginning to the end of your visit. We can already see the benefits in time and cost saving of these new systems for us and for you.

We are aware that this process has caused some confusion, especially with phone and email communication and sincerely apologize for not being more timely with updates during the installment process and the whole transition period.

If you happen to receive message reminders that are not from SMCH, requesting that you cancel, confirm, reschedule, or re-register your appointment, please note that you do not need to take any action. These are residual effects of the earlier collaboration process and this should no longer be an issue by the first week of August.

If you still have questions or concerns, we encourage you to call our office directly and we will resolve the issue for you. You may also contact us by email -

Attention - Tracey or Mariana

Thank you for your patience during these recent challenges. Those smiles, hugs, text messages, emails, cards, food and reminders to "breathe" from so many of you kept us committed and strong.

The SMCH team is working diligently to ensure a smooth and satisfying experience that focuses on you and those you love.

We are ever grateful and honored by your continued trust in making us your dedicated medical home.

With great hope and all of our heartfelt appreciation,

Tracey and The Team at South Mountain Community Health

Important Community Message !

The power is out at the office and the surrounding areas. We don't know when it will be repaired. If you have an appointment, the staff will be reaching out to see if you have an immediate need that needs to be addressed or you can be rescheduled. If you need to reach the office, our telehealth line can be texted at 301-822-2925. Someone will return your call.
Thank you. Stay safe! updates will be posted here.

Good morning dear Community,
I hope the sunny weekend was a good exchange for losing a precious hour of sleep. Spring is definitely teasing us.
Just giving you a quick heads up that our dear Quest phlebotomist has spent the weekend very under the weather and sent her (big) regrets to "my great patients" that it would be best if she stayed home an extra day. We will check on her all day and encourage her to take care of herself the way she takes care of all of us. If you don't hear back from me, the lab will reopen on Tuesday at 730 am.
You still make me hopeful every day,
Tracey

Good morning South Mountain Community!

Wishing you a safe and happy day of ghoulies, goblins and vampires.
Speaking of vampires......have you all met our wonderful and gentle phlebotomist, Liberty? For your convenience we have given space to Quest lab right in our office for over a year now. There is never a wait and she will help you get signed up for the Quest portal version of "my chart" so you can have easy access to your results. Quest takes all insurances and the fees are no different from any other labs. For the faint hearted, Liberty will also come out and meet you in one of the exam rooms to make the experience even easier.
I know she has gained many fans and I hear you all greeting her as you come in and out all day long. You do not have to be a patient of the practice to use the lab and you may also bring orders from any of your specialists. And don't forget that we have an elevator. If you ring the bell, she will even come down to get you!
We are all so grateful that she has joined our crew and hope that you will consider having your next lab draw with her. We want Quest to keep her here!

The lab hours are -

Monday, Wednesday - 8 am - 430 pm (last draw at 4)
Tuesday, Thursday - 7 am - 3 pm (last draw at 230)
Friday - 7 am - 12 pm (last draw at 1130)

Wishing you peace and hope always,
Tracey

Good morning my beloved Community,
As the days just fly in and out, catching up on social media has not ever been my greatest strength, but I could not let the month pass without marking a huge celebration for South Mountain Community Health. After nine years of getting to know many of you in my first practice, I took a chance on my vision of the way health care should be delivered and opened South Mountain Women's Health in a tiny office on the the second floor of our current building. You not only came, you then brought your partners, parents, children, friends and neighbors. YOU are the reason we are still here - 5 Nurse Practitioners, 9 wonderful Support Staff and an in-house LAB with our QUEST partners. YOU took us from that tiny women's health practice and put the COMMUNITY in our name and in our mission.
I am filled with love and gratitude for the faith you have shown in us. We are grateful for your patience and support every day as we resolve old growing pains and run into new ones. And we are committed to continue to learn, evolve and improve with you and with our Community through the next 200,000 visits as we did the last 200,000.
I am celebrating YOU, the South Mountain Community today and every day.
Happy Anniversary!
I am filled with great hope for another 20 years with you.
Always,
Tracey
