Proud of the incredible work and presentation by Danny Gentile and Kellie St. Martin representing Upham’s Community Care at the Community Health Institute.

Danny and Kellie highlighted how our team expanded telehealth video visits by applying lessons from the technology and design industries to improve the patient experience and increase access to care. Their work helped drive a 28x increase in utilization while improving patient engagement and appointment completion rates — especially for patients who face barriers leaving home.

Their presentation reflected the innovation, collaboration, and patient-centered focus that define Upham’s. Thank you both for representing our organization and sharing this important work with community health center leaders from across the state.

How do we best reach patients who have challenges leaving home? At CHI, Upham's Community Care demonstrated how they took on this challenge and increased their telehealth video visits by 28 times!

Drawing on lessons from the tech and design industries, Upham’s shifted their telehealth model. The results? A dramatic increase in patient access, deeper patient engagement, and higher appointment completion rates.

We’re proud to announce the launch of our Family Nurse Practitioner (FNP) Residency Program!

The Upham’s Community Care residency is designed to support newly graduated Family Nurse Practitioners as they transition into confident, skilled, and compassionate primary care providers. Through structured mentorship, hands-on clinical training, and interdisciplinary collaboration, residents will gain advanced experience in family practice and community health, caring for patients across the lifespan.

Our FNP Residency offers:
• Dedicated preceptors and one-on-one mentorship
• Comprehensive primary care and community health training
• Experience in population health management
• Professional development and leadership growth
• A supportive, team-based learning environment

We are committed to strengthening the primary care workforce, improving community health outcomes, and investing in the next generation of Family Nurse Practitioners.

Upham’s Community Care was proud to stand alongside the Massachusetts League of Community Health Centers on Beacon Hill today. Our CEO, Jay Trivedi, provided testimony highlighting the urgent need for sustainable and equitable commercial reimbursement rates for community health centers.

As Jay noted, chronic underpayment from commercial payers undermines access to care, limits workforce retention, and strains the very systems designed to serve our most vulnerable populations. Establishing a PPS floor for commercial payers would be a critical step toward stabilizing operations and ensuring health access for all Massachusetts residents.

We thank the League and our fellow community health centers for their leadership and collaboration in advancing this essential policy effort.

Upham’s Corner Health Center’s team observes National Recovery Month and celebrates UCC’s patients and the millions of Americans who are in recovery. To celebrate Recovery month this year, we have dedicated time to promote and open up discussions surrounding recovery, increase public awareness and reduce stigma. Our calendar of events have included guest speakers with lived experience and insight:

September 5; Faces in Recovery, a Q&A event with two guest speakers in recovery, and opportunity for staff to bring any questions to speakers and explore better ways to serve individuals with substance use disorders.

September 19; A conversation with Danielle O’Brien, Director of Recovery Support Services at BSAS. Danielle will share her journey in long-term recovery and how to facilitate patients access to recovery support services. She was joined by recovery coaches from North Suffolk Community Services, Torchlight Recovery Cafe

Throughout the month of September, our SUD team will be sending out SUD informational flyers to UCC’s staff and using interactive games to share Recovery month information, and engage in one-to-one conversations with staff and patients on why Words Matter when we are talking about substance use or a person with a substance use disorder. We will also be inviting staff and patients to sign the Words Matter Pledge, to promote UCC’s goal of creating a stigma-free community health center environment.
