Upham's Community Care Upham’s Community Care offers medical, dental, vision, elder, and urgent care; WIC; home health care; UCHC is a registered 501(c)3 nonprofit organization.

Since its inception in 1971, UCHC has been committed to offering high-quality, affordable primary health care for all ages, races, and genders. Today, UCHC offers a wide range of services, including family medicine, women’s health, behavioral health, nutrition, elder care, urgent care, dental, vision care, WIC, PACE, and home health care. This care is tailored to meet the cultural, financial, language, and access needs of a vibrant and diverse community. Boston health care partners include Boston Medical Center, Mass General Brigham, and Boston Children’s Hospital.

🎉 It’s National Health Center Week!

We’re proud to celebrate 60 years of the community health center movement—a movement that began right here in Dorchester at Columbia Point Health Center in 1965.

Just a few years later, Upham’s was incorporated in 1971 to continue that mission of bringing care directly to the community. For over 50 years, we’ve served our neighbors with health care, behavioral health, elder care, and support services—right where people live.

Uphams Community Care is proud to partner with the Boston Public Health Commission in addressing the opioid crisis.

Thanks to this collaboration, and the leadership of Mayor Michelle Wu 吳弭 and Dr. Bisola Ojikutu, we’ve seen major progress—Boston reported a 38% drop in opioid-related deaths.

For over 50 years, our mission as a community health center, PACE, and home health provider has been to deliver compassionate, high-quality care to all who seek our services. This commitment has been the foundation of everything we do.

These are challenging times, and the events of the past week reflect the uncertainty many are experiencing. Yet, no matter what changes may come in policies or politics, our dedication remains steadfast. We continue to provide exceptional care to all our patients while ensuring access to essential resources and support for those in need.

Need help with signing up for Medicare? Want to review your current plan? Stop by 415 Columbia Road on November 19th from 10:30 am-3:30pm! SHINE Counselors (Serving Health Insurance Needs for Everyone) will be happy to assist you with Medicare enrollment and to answer any questions you may have.

Spots are limited. Please contact 617-635-4366 prior to November 19th to schedule your appointment for the open enrollment event. Be sure to bring your list of prescriptions, Medicare/other insurance cards, your Medicare.gov username/password (if already set up), and valid ID to your appointment.

Thank you to Boston's Age Strong/ Age-Friendly Commission for stopping by our Roxbury PACE center last week! To ensure Boston is the best place for elders to live, the Age Strong representatives asked our PACE participants to give feedback about what they love about living in Boston, what they would like to see more of, and what can be approved.

Do you have ideas about how to make Boston a more supportive city? Age-Friendly Boston would love to hear from you! Age-Friendly Boston is currently updating its five-year plan and is looking for residents around Boston for ideas and suggestions to improve the city. They are conducting listening tours around Boston to solicit input.

The Dorchester Food Co-op visited PACE again this year, offering fresh, local produce to participant & staff free of charge. Produce included melons, apples, carrots, beets, bok choy, collard greens, cauliflower, and cranberries.

In total, 61 participants engaged in the Pop-up Farm Stands at 1140 and 36, and roughly 25 staff members.
