💙This month, we are focused on building stronger relationships with local physicians to better support patients at home.

💙Many seniors and adults living with chronic conditions need more than a discharge summary — they need consistent, hands-on support.

Chronic conditions impact the majority of adults in the U.S., and they are the leading cause of hospitalizations and readmissions.

🔷Strong partnerships mean better outcomes for families.

If you’re a physician, case manager, or family member looking for coordinated support, we’re here to help.

📌 Why Doctors Often Refer Patients to Home Care

Chronic conditions are increasingly the norm in our communities, not the exception:

📊 In 2023, an estimated 76% of U.S. adults had at least one chronic health condition, and more than half had multiple chronic conditions. Chronic conditions like heart disease, diabetes, COPD, and hypertension are major drivers of illness and healthcare utilization. (CDC,2025)

🔹 Chronic conditions fuel healthcare costs — about 90% of U.S. healthcare spending is for people with chronic and mental health conditions. (CDC, 2025)

🔹 Hospital readmissions remain a significant issue — many patients with chronic illnesses return to the hospital within 30 days of discharge, representing preventable stress on patients and the system.

Doctors refer to home care because it:

✔ Helps patients manage complex medication regimens
✔ Reduces risk of complications at home
✔ Supports daily living needs and safety
✔ Improves follow-up care after a hospital stay

Home care isn’t an add-on — it’s an evidence-based step in keeping patients healthier at home and out of the hospital.

💙 Caring for chronic conditions well means better outcomes and greater independence.

Solomon Carter Fuller

Solomon, a psychiatrist, researcher, and medical educator, was born on August 11, 1872 in Monrovia, Liberia. Solomon Carter Fuller graduated from Boston University School of Medicine with his M.D, where he would later become a faculty member. He went on to further research at the University of Munich, Germany, where he researched neuropathology and was 1 of 5 foreign students chosen by Alois Alzheimer studying the traits of dementia.

After returning to the United States, he continued his research on what would later be known as Alzheimer’s disease and other mental illnesses such as schizophrenia and depression. Fuller published the first comprehensive review of Alzheimer’s disease in 1912 and is known best for his groundbreaking research on the brain’s physical changes that occur in Alzheimer’s patients.

In 1974, the Black Psychiatrists of America created the Solomon Carter Fuller Program for young Black aspiring psychiatrists to complete their residency. The Solomon Carter Fuller Mental Health Center in Boston is also named after Dr. Fuller.

February is Black History Month – A Time to Raise Awareness About Heart Health in Black Communities

Black History Month is a time to reflect on the achievements, contributions, and challenges faced by African Americans. One of the most pressing health issues affecting Black communities is heart disease, which remains the leading cause of death among African Americans. At Warm Embrace Home Care, we are committed to addressing heart health disparities and ensuring that everyone has access to quality in-home care to help prevent and manage cardiovascular conditions.

Heart Health Disparities in Black Communities

Heart disease impacts African Americans at higher rates compared to other racial and ethnic groups. Key statistics highlight the urgency of this issue:

📌 African Americans are 30% more likely to die from heart disease than non-Hispanic whites.

📌 Nearly 47% of Black adults have high blood pressure (hypertension), a leading cause of heart attacks and strokes.

📌 Black individuals experience higher rates of obesity and diabetes, which contribute to cardiovascular disease.

Addressing these disparities requires awareness, prevention, and access to care, and that’s where Warm Embrace Gome Care steps in.
