Chestnut Knoll Personal Care Community was officially opened at 120 West 5th Street, Boyertown, PA 19512, on September 18, 2000. This facility is licensed by the Department of Public Welfare as a personal care home. Chestnut Knoll has 86 suites, 34 of which are located within our secured dementia care unit, area devoted to memory-impairing illnesses such as Alzheimer's Disease.

The FOX Awards are one of our favorite traditions at Chestnut Knoll. The ceremony gives us the opportunity to recognize residents whose effort, determination, and positive spirit inspire everyone around them. It also allows us to showcase how our partnership with FOX Rehabilitation and our unique FOX Optimal Living Program makes Chestnut Knoll a place like no other!
