First and foremost, we would like to congratulate Dr. Elliot Michel of Valley Neurology, who will be retiring this upcoming April 1st.
NeuroPitt will become the custodian of the patient charts from Valley Neurology.
Dr. Matthew Kozminski will continue to operate outpatient clinics under this own practice in Brackenridge and Kittanning, thus, you may consider transitioning your chart and care to his practice.
Bear in mind that Dr. Kozminski will not be operating a clinic in New Castle, thus, if you are a Valley Neurology patient from the New Castle region you are advised to begin a search for a new neurologist at this time.
In order to transition your care to another neurology practice as smooth as possible, it is highly recommended that you take the following steps:
1) Make a new patient appointment with another neurologist. Then call 724-749-4188. A staff member will gladly fax your records to your new neurologist.
2) If your new neurologist asks you to sign a release of records, have that office fax the form to: 724-202-0394. Once received, a staff member will gladly fax your records to the appropriate office.
3) If you desire to have the records released to yourself, bear in mind that due to HIPPA regulations, we cannot email or fax records to your personal email/fax. You must sign a release of records to yourself in person and pick up the records. Bear in mind that you will be charged a fee to release records to yourself.
