Visiting Angels is looking to hire a Part-Time Nursing Supervisor/ Director of Nursing.

Due to COVID-19, Many of the below items are being conducted virtual/ via phone until current executive orders are lifted/ expire.

Position Details:

Responsible for the supervision of the Nursing Supervisors, Field Nurses, Certified Home Health Aides and Companions

Responsible for the initial in-home evaluation of the client

Perform in-home visits or telephone consultation during both normal business hours and off hours as needed

Initiate the plan of care and make necessary revisions

Assure that the client and family understand the home Plan of Care and their responsibilities

Perform case orientation to Home Health Aides

Perform 30 day follow up calls to clients

Conduct in-home supervisory visits in accordance with agency guidelines and/or state licensure regulations

Conduct orientation and training in accordance with the agency guidelines and regulatory standards

Conduct clinical competency assessments and evaluations as needed

Evaluate clinical components of performance evaluations

Supervise, teach and orient caregivers per agency guidelines and regulatory standards

Manages the Nursing on-call schedule and assumes on call responsibility on an as needed rotating basis to manage unexpected client clinical needs that occur after office hours

Maintain the PQI Program & ensure all reporting requirements are met.

Participate in the quality assurance reviews and evaluations of the services provided to the clients.

Develops and coordinates Performance Improvement activities to achieve quality outcomes and compliance with standards

Facilitates corrective action plans in response to Performance Improvement outcome measures and inspection surveys.

Receives and investigates service/caregiver complaints and unusual occurrence reports to resolve issues; and submits reports, investigative outcomes, and responses as indicated to the Director of Operations.

Monitors complaints and unusual occurrence reports to identify prevalence/frequency of issues and trends for performance improvement opportunities

Provide excellent conflict and complaint management and resolution

Maintain a safe environment for clients

Manage and coordinate the care of clients with the caregiver and maintain a pleasant and cohesive working relationship.

Ensure compliance with legal, regulatory and accreditation requirements

Demonstrate knowledge of current health and home care practices

Perform related duties and responsibilities as deemed appropriate by the management team

Demonstrate dependability

Plan, organize, prioritize, delegate and accurately follow through in work activities with time constraints and interruptions to meet deadlines, as well as, work independently with a minimum amount of direction and/or supervision

Ensure coordination with other departments, services and senior management, as appropriate

Ensure that appropriate service policies and procedures are developed and implemented to accomplish identified outcomes

Direct staff in performance of their duties including admission, discharge, transfer, revocation, and provision of service to patients.

Ensure appropriate staff supervision during all service hours

Ensure standards of ethical business and clinical practice are maintained

Meet all medical requirements in accordance with agency guidelines and/or state regulations

Ensure all required medical requirements are current

Assist with transporting caregivers as necessary
