Job Title: Case Manager
We are looking for a Case Manager to enhance the quality of clients’ waiver services management, maximize satisfaction and promote quality outcomes. The successful candidate will be accountable for the full participant care cycle. Case Manager duties include assessing, planning, implementing, monitoring, and evaluating actions required to meet the client’s health and human services needed.
Responsibilities:
- Provide supervision to all direct service workers in their assigned caseload to ensure that quality care is being provided to recipient
-Responsible for training direct service workers on their recipient's individualized plan of care and timesheet before going into the home and as needed when changes occur
-Responsible for training new DSWs and/or currently contracted DSWs on recipient ISP, timesheet, and service log/progress notes. Case Manager must show proper documentation of training, and must be approved by office manager before the contractor is allowed to start
-Responsible for conducting face to face visits with every recipient in their caseload every three months and conduct one pop up visit annually. You are to keep making pop up visits until QA manager has a successful visit.
-Must submit all quarterly and monthly forms to QA supervisor to sign off on before filing in recipient's binder
- Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
- Develop effective working relations and cooperate with the interdisciplinary team throughout the entire case management process
- Take the extra mile and interact with clients to keep track of their progress and to ensure satisfaction
- Adhere to professional standards as outlined by protocols, rules, and regulations
Skills:
- Proven working experience in case management or a related field preferred
- Previous experience with developmental disabilities preferred
- Effective communication skills
- Excellent organizational and time management skills
- Problem-solving skills and ability to multi-task
- Compassionate with teamwork skills
Qualifications:
-High School Diploma or Equivalent
- Current driver’s license
- 2 years of Case Management Experience Preferred
- Knowledge in HCBS (Home and Community Based Services Rules and Regulations) (Preferred)
