If you need to get a message to anyone at the Haralson location you can call Highland Rivers scheduling at 1-800-729-5700 chose option '0" and have a message sent to the staff member you need to.

Community Support Specialist, Bremen
Description
Community Support Services (CSS) are medium-intensity community-based services that promote mental health recovery for people with persistent mental health issues. Services are customized to the needs and preferences of the individual. CSS helps restore individuals to their best possible level of functioning, while building the independence, resiliency and coping skills needed to live in their communities.

Job Responsibilities include:
Community based position.
Assist individuals with service needs, developing strategies and supportive, social support networks and coordinating rehabilitation services as specified in the consumer’s individual recovery plan.
Partner with the individual and CORE services provider in the development of the individual service plan.
Assist the individual with the transitioning between levels of care. Link and/or serve as advocate with other agencies to garner needed resources for consumers.
Provides assistance and support in crisis situations.
Provides assessment, monitoring, and assistance to individuals.
Establishes professional and ethical relationships with individuals, families, and coworkers
Provides direct service to individuals; completes all necessary paperwork according to policy; ensures individual meet eligibility criteria or refers to appropriate provider in the community.
Manages a caseload by ensuring all services are authorized, chart paperwork is complete and current, and consumer charts are closed according to Policy & Procedure.
Sees a caseload at the frequency defined in the service plan; documents attempts to contact individuals and ISP reviews; completes monthly caseload reports; track appointments and case management.

Requirements
Bachelor's Degree in Helping Field
At least one year’s experience working in mental health or substance use disorder field preferred.
Must be able to pass background investigation, drug screen and have good 7year motor vehicle report.
Bi-lingual abilities a plus

HPI will be cooking a spaghetti dinner on Friday September 17th, serving at 11:00 a.m. Each plate will consist of spaghetti, corn, garlic bread and a dessert, along with a drink. We are asking for a $7.00 donation for each plate sold. The proceeds will benefit activities and special outings for HPI individuals. It is with great sadness that we announce the passing of our very own Beth Summerville. Beth has worked for HBHS for 21 years. She was a friend to everyone, a passionate advocate for the mentally ill and loved by all who knew her. She will be greatly missed. Please keep her family in your prayers.
