Monogram Health is thrilled to announce our partnership with Aetna (a CVS Health Company) has expanded to Florida. Now, all eligible Aetna Medicare Advantage members across the state, who meet the program’s criteria, will benefit from Monogram’s value-based, in-home, multispecialty care for patients suffering from polychronic disease.

Like many states, Florida faces a shortage of healthcare professionals, rising healthcare costs, and the need to expand interventional services. Monogram Health has a proven track record for driving positive clinical outcomes, decreasing hospital and ER admission rates, and increasing star ratings, resulting in reduced medical costs and a high quality of living for all engaged patients.

To date, the CDC reports that kidney disease, heart disease, diabetes, and lower respiratory disease are among the top 10 leading causes of death in Florida. Along with Monogram Health’s patient-centric, multispecialty treatment plans, eligible, Florida-based Aetna patients (nearly 13,000) will have 24/7 access to personalized care from doctors, nurse practitioners, specialists, pharmacists, registered nurses, and social workers all in one place. Monogram Health treats and diagnoses health issues; reviews and prescribes medication; provides guidance, education, and counseling on patients’ healthcare options; as well as assists with daily needs such as access to food, eating healthy, transportation, financial assistance, and more.

Since 2023, Monogram Health has successfully collaborated with Aetna to care for more than 50,000 attributed patients across Iowa, New Jersey, Pennsylvania, Texas - and now Florida - with a goal of expanding to more states shortly.
