Proud to have sent our colleagues and to attend the Conference this November in National Harbor, Maryland. As we continue to listen and engage with complex, polychronic populations across the country, it is important to understand how we can better serve and deliver treatment and care to patients enrolled in Medicaid, SNP, and Medicare Advantage programs.
This year’s conference was focused on expanding access to care, and Monogram is proud to be at the forefront of this initiative as the leading in-home, multi-specialty provider for aging patients with multiple chronic conditions. Monogram’s model brings trusted clinicians directly to patients’ homes, including rural communities.
