March 23-24, 2026, Monogram Health gathered 30+ leading physicians and specialists from across the country to Nashville’s Southall Farm & Inn to break down clinical models and discuss what it means to deliver multispecialty treatment to patients who need it most. The 2026 meeting focused on pulmonary and its role as a force multiplier in patients with other chronic illnesses. This elite group was represented by several specialties across cardiology, endocrinology, pulmonology, nephrology, behavioral health, family practice, and palliative care.

In the U.S., March 30th is set aside to appreciate physicians, their respective skillsets, and their commitment to patient treatment and care. We are beyond grateful to our 62 multispecialty physicians who are truly putting the patient first as they simultaneously evaluate, diagnose, and treat the whole person (and all chronic conditions present). Earlier this week, Monogram Health gathered 30+ leading physicians and specialists to Nashville’s Southall Farm & Inn to break down clinical models and discuss what it means to truly deliver polychronic treatment to patients who need it most. Although this year’s National Clinical Advisory Board meeting focused on pulmonary care, and its role as a force multiplier in patients with other chronic illnesses, the group was represented by several specialties across cardiology, endocrinology, pulmonology, nephrology, behavioral health, family practice, and palliative care.

Today, the 3rd article in Monogram Health’s polychronic series was published in !

In his article, “Reinventing Pulmonary Care for Medically Complex Patients in America,” Dr. Lakshman Swamy MD, MBA, FCCP discusses the interactions between pulmonary and other polychronic conditions that patients face every day. He goes on to explain that the impact of pulmonary diseases on other chronic conditions is often misunderstood and commonly underreported. In fact, more than half of Americans with Chronic Obstructive Pulmonary Disease (COPD) have not been diagnosed.

Throughout the article, he highlights that the interplay between chronically dysfunctional organ systems is so profound that a siloed, fragmented approach to healthcare makes it impossible to focus on what matters most - the patient’s overall health, personal goals, and the social determinants of health that impact them. In other words, a siloed approach to sub-specialty care misses seeing the patient as a whole person.
