Mr. Thomas Hutto lives on his own in Garrison, Texas (population 846). Mr. Hutto has advanced COPD, congestive heart failure, and CKD. Before Monogram, Mr. Hutto had to travel 2+ hours to see a specialist or visit the nearest “walk-in only” Urgent Care for any exacerbations he was experiencing or to get his medications refilled/reviewed. Since Monogram, Mr. Hutto has been able to stay in his home and receive the multispecialty treatment and care he needs on his time. He now has direct access to clinical tests, pharmacists, nurses, doctors, and specialists 24/7 at his kitchen table.

May is Mental Health Awareness Month. Mental health conditions are highly prevalent across the populations we serve, particularly among patients with multiple chronic conditions. More than 1 in 5 U.S. adults experience a mental illness each year, and nearly 1 in 3 adults with a chronic medical condition also have a co-occurring behavioral health condition such as depression or anxiety. Individuals with polychronic disease carry a disproportionate burden of these challenges, with depression rates 2-3 times higher than in the general population.

This May, we acknowledge this reality and continue to treat and care for the whole patient 24/7.

Today, the 5th edition of Monogram’s Multispecialty Series in dropped!

In the article “From Slowing Progression to Remission: The Quiet Nephrology Revolution,” Dr. Mindy Banks explores how Monogram Health is moving beyond the historical goal of delaying the inevitable loss of kidney function. Monogram’s high-touch, data-rich model has shown that dealing with kidney health means changing the approach from merely slowing down the progression of kidney disease to possible clinical remission.

The complex polychronic nature of our aging patient population requires that we not focus solely on one organ or disease, because every organ relies on the others for optimal functionality. In fact, Monogram’s proactive approach can be extremely impactful, as approximately 14% of the U.S. adult population has chronic kidney disease, but the vast majority of people die of something other than their CKD.
