EverTrue is HIRING! Primary Care Physician (PACE Program)

About EverTrue PACE
The EverTrue Program of All-Inclusive Care for the Elderly (PACE) is a proven, cost-effective model of care that allows older adults to remain safely in their homes while receiving comprehensive medical and social services. Care is delivered by an interdisciplinary team of healthcare professionals who support participants’ physical, emotional, and social well-being—both at home and within the EverTrue PACE Center.

Position Summary
The Primary Care Physician provides primary and continuous medical care to PACE participants. This role requires strong clinical judgment, collaboration within an interdisciplinary team, and a commitment to caring for a frail elderly population in accordance with PACE program standards.

EverTrue PACE Services
• Medical Care: Preventative, chronic, and acute care management
• Therapy Services: Physical, occupational, speech, and recreational therapy (home-based or onsite in a state-of-the-art therapy gym)
• Mental Health Support: Counseling and crisis intervention in collaboration with social work services
• Case Management: Regular assessments and care plan updates based on changes in health or lifestyle
• Dining: Nutritious, chef-prepared meals that balance health with enjoyment
• Transportation: Door-to-door transportation to and from the PACE center and outside medical appointments
• Personal Care: Assistance with activities of daily living, housekeeping, meal preparation, and access to onsite showers and laundry

Qualifications, Knowledge, Skills, and Abilities
• Medical degree (MD or DO)
• Current Missouri Physician License
• Minimum of two (2) years’ experience working with an elderly population
