This donation is truly so meaningful to our program. I was moved to tears accepting this donation from Sam knowing that she will carry CPDC and its impact with her for years to come ❤️

When Sam Smith, '25, crossed the stage at Gillette Stadium, she was handed $1,000 in cash with a simple instruction: Keep $500, and donate $500 to a cause she deemed worthy of support.

Sam walked directly over to the Children's Physical Developmental Clinic and handed over the envelope to fulfill her philanthropic mission.

Please note this program is for families who have MassHealth STANDARD

Deadline to submit your application is November 1st!

The Autism Division of the Department of Developmental Services (DDS) administers the Children’s Autism Waiver Program (AWP) which provides one-to-one interventions to help children with autism who exhibit severe behavioral, social and communication challenges through a service called Expanded Habilitation, Education (intensive in-home services and supports).

The forms are also available in Arabic, Brazilian Portuguese, Cape Verdean Creole, European Portuguese, French, Haitian Creole, Nepali, Somali, Spanish, Tradition Chinese, Turkish and Vietnamese.
