Knowing your HIV status is one of the most powerful steps you can take for your health — and for the health of our community. HIV testing is quick, confidential, and available right here at the Brockton Neighborhood Health Center.

Please join us for an engaging webinar on 12/2 at 12pm showcasing how Brockton Neighborhood Health Center (BNHC) is harnessing data to improve the care of perinatal patients.

Participants will learn how the BNHC team collaborated with Community Technology Cooperative to create Epic-based tools and dashboards that simplify clinical workflows and empower OB providers to identify high risk patients, guide outreach, close care gaps, and advance health equity.

The keynote speaker will be Jeanie Jeudy, RN, MOM’s Do Care (MDC) Program Lead, Brockton Neighborhood Health Center (BNHC). MDC provides nurse case management, perinatal recovery coaching, care navigation, midwifery, and behavioral healthcare.

This session will be moderated by Joe Panerio-Langer, MD, Deputy Chief Executive Officer at Brockton Neighborhood Health Center.
