You may be wondering why we are asking you to fill out an intake form and look over and sign our client policies….

✨We are licensed and there are many things we learn through schooling about medical diagnoses and medications that contraindicate massage on many levels so we do this to protect you as well as ourselves!

✨You can provide information on what your typical problem areas are and we always ask of new ones before each session so we can tailor each session to your needs!

✨For the policies: we do this to help keep our expectations for clients and ourselves transparent. Having this written out and agreed upon before you’re established as a client keeps the confusion low and prevents it from seeming like we make the rules as we go!

We appreciate you taking the time to fill these forms out and will have you do a new intake form once yearly so that all your information stays up to date, but if there are ever any changes to your medical diagnoses or medication list, please inform us so that we can continue to keep all of our clients safe!
