All W2 forms that aren’t picked up before the last week of January will be mailed out; if you have failed to report a change of address thru a W4 the agency is not responsible for lost or stolen forms as well as the agency does not control USPS

failing to do so may result in losing your provider services

If you need to fax it we can assist you with that so you don’t spend money on fax services

To our employees: remember it is part of your duties to inform of any Medicaid issue or change to the client you serve
