A better quality of life begins at Plum Creek Assisted Living.

Our approach is different. As a family-owned and operated community, we are proud to be the area’s only true standalone assisted living community. Since 2009, we have been honored to serve seniors and families throughout Medina County with compassionate care, personalized support, and a true sense of home.

At Plum Creek, residents enjoy single-story living in private apartments, each with a private bathroom and a kitchenette complete with a refrigerator and microwave. Our community is staffed 24/7 with dedicated nurses and resident assistants, supported by an incredible multidisciplinary team that helps provide an added level of care and comfort.

Residents also enjoy a beautiful dining room with home-cooked meals, a vibrant activities program that encourages social connection, organized exercise classes, spiritual wellness services in our chapel, and the support of a full housekeeping and laundry team. We are also proud to have an outstanding Medical Director with privileges at Cleveland Clinic Medina Hospital.

From our private patio to our enclosed courtyard, Plum Creek offers beautiful outdoor spaces designed for relaxation and enjoyment. We also offer the peace of mind of no long-term contracts, with residency on a simple 30-day basis. Medicaid Waiver may be available after one year of private pay, based on availability.

From Resident Assistant to ICU Registered Nurse—Nathan’s journey is a powerful example of dedication and growth. Starting right here at Plum Creek, he advanced to LPN, then RN, and is now working toward his BSN while serving in the ICU.

Beyond his clinical work, Nathan is giving back by volunteering with The HANDS Foundation, helping educate and connect with local seniors in community centers and independent living communities—and is even making sure our residents have all the education they need, too!

We’re also proud to share that his educational journey will be featured in the next HANDS publication.

Education is power—and Nathan is putting it to work every day. We love you, Nathan, and we are so proud of you!

Deciding on assisted living—for yourself or someone you love—is never easy. It’s a deeply personal choice, and it can feel overwhelming, especially when you’re making that decision for someone else.

One of our Resident Assistants took a moment to thank the entire team after a particularly busy shift—and it says everything about who we are. From a trainee stepping up, to a team member staying late, to leadership jumping in on a Saturday evening—everyone showed up. How often to do you see a Community Relations Director and Director of Nursing doing resident care?!
