Community Health Professionals (CHP) Home Care & Hospice of Bryan is a non-profit home health & hospice organization serving the Williams County area.

What makes CHP unique is that we provide a complete range of services - including visiting nurses, in-home therapy, hospice, inpatient hospice, private duty home care and adult day services.

Our nurses and staff who are local and available 24-hours a day, seven days a week. Making a referral to CHP is easy...just call us! Our staff will help you decide what service is right for you and we will work with your physician. We are happy to provide a no obligation, informational visit by one of our friendly and caring nurses. She will be happy to answer any questions you may have.
