As the regulations surrounding COVID-19 are coming to an end, Adapt gets a head start. Some of our patients will be joining us in office for their annual physical exam to ensure that their care can continue undisturbed. Most of our other patients may continue their care exclusively virtually as they always have. Where many mental health programs will struggle to ensure continuity of care in a post-Covid era, Adapt stays several steps ahead.

The Role:
Seeking a New York State licensed and board certified family or adult nurse practitioner for a fully remote/ work-from-home telemedicine opportunity. The assignment involved telephonic assessments of recently discharged patients to evaluate their post-acute needs. All training is provided.

This is a contracted 1099 position offering flexible scheduling hours and bi-weekly direct deposit payments for services rendered.

The Qualifications:

-Masters Degree (or higher) from an accredited institution
-Valid New York State FNP or AGNP license
-Board Certification in Family or Adult/Gerontology
-New graduate nurse practitioners welcome to apply but 1 year telehealth experience strongly preferred
-Subacute nursing experience is a plus

In this role, you will provide diagnostic and therapeutic services to patients in a long term care setting; coordinating with facility staff as well as specialty medical services to deliver high quality care to an aging population. This is a flexible and autonomous role that creates enormous satisfaction for the NP as you impact the care and comfort of patients who will inevitably develop a close relationship with you. This is a smaller than typical long term care setting in which you will work together with a physician, physical therapy-occupational-dietary professionals, and a dedicated nursing team. All charting is done electronically.

Requirements:
-A NYS licensed and board certified family OR adult nurse practitioner.
-Have at least 1 year of experience in a long term care/subacute rehab setting.
-Hold an active DEA license and have prescriptive authority
