The Community Health Center of Buffalo, Inc. is a Federally Qualified Health Center (FQHC) that provides access to quality and affordable health care to all members of the community regardless of their ability to pay. In 1999, the Community Health Center of Buffalo, Inc., opened its doors at 465 Grider Street, Buffalo, New York on the grounds of the Erie County Medical Center Since 1999, the Community Health Center of Buffalo, Inc. We're more than just a health care center. We are part of the Community! We have been caring for our neighbors, your friends and family for over 18 years. We are here for you when you are sick and well! Providing You with Patient-Centered Medical Care We believe it is important for each patient to take control of their health and wellness by establishing a relationship with a primary doctor because it helps in the early detection of life threatening diseases. We strive to not only treat your illness during a visit but provide you with the information needed to manage any chronic disease or condition you may have like diabetes or high blood pressure. In December, 2018, we added physical therapy to our menu of service offerings. Our physical therapy space offers state of the art equipment and highly qualified physical therapy staff.
