The Community Health Center of Buffalo, Inc. is pleased to share that Dr. Shahla Ahmad, MD, Family Medicine/Geriatric Medicine, and Dr. Ann-Marie John, PhD, MSN, director of Prevention and Wellness, presented at the Healthcare Association of New York State, Inc. (HANYS) Age-Friendly Conference on November 20.

Their presentation, Addressing Multicomplexity and Multimorbidity in Age-Friendly Geriatric Care at Community Health Center of Buffalo, Inc., highlighted how our team is leading the way in caring for older adults with multiple chronic conditions, unique social needs, and functional challenges.

As Age-Friendly Health Systems evolve, many organizations including ours are adopting the 5th M: Multicomplexity to better capture the full picture of what older adults experience.

Dr. Ahmad and Dr. John shared Community Health Center of Buffalo, Inc.’s approach to:
• Coordinating care across clinical and community partners
• Managing co-occurring health conditions

We’re grateful for our incredible medical providers and talented team of staff, for our patients, and our community partners. Wishing everyone who celebrates a healthy and Happy Thanksgiving! May your day be filled with joy, good health, and a year full of gratitude.

Please note that all Community Health Center of Buffalo, Inc. locations (Buffalo, Cheektowaga, Niagara Falls, and Lockport) will be closed November 27 and 28 to give our staff time to be with family and friends. Our 34 Benwood Avenue location in Buffalo, NY will be open on Saturday, November 29, from 8 a.m. to 5 p.m. All locations will be open for normal business hours on Monday, December 1.

💜 The holidays are about being together with family and friends – Alzheimer’s and dementia does not need to change that. With some planning and adjusted expectations, holiday celebrations can be as enjoyable as possible for your loved one with Alzheimer’s or dementia-related illnesses. Here are some tips ⬇️ Learn more from the Alzheimer's Association:

• Plan a group discussion for family and friends to discuss holiday celebrations in advance. Update them on changes they may see in the person living with Alzheimer’s/dementia. Let them know they can help by being patient, not interrupting or correcting, and giving the person time to finish their thoughts. If the person cannot communicate clearly, respond to their tone.

• If you’re hosting, consider having a potluck meal or asking someone to order and bring dinner.

• Celebrate by having a lunch or earlier dinner, rather than an evening meal, so you can work around evening confusion (sundowning) if it sometimes affects the person living with Alzheimer’s/dementia.

• Build on traditions and experiment with new ones that might be less stressful or a better fit with your caregiving responsibilities, such as watching holiday movies.

• Involve the person with Alzheimer’s/dementia in safe, manageable preparation activities that they enjoy. For example, ask them to pack cookies in tins, fold napkins and towels, or help decorate. Do not use artificial candies, fruits, and veggies as decorations because a person living with Alzheimer’s/dementia might confuse them with real food. Blinking lights may cause confusion and unnecessary irritation.

The Community Health Center of Buffalo, Inc. is a Federally Qualified Health Center (FQHC) that provides access to quality and affordable health care to all members of the community regardless of their ability to pay. In 1999, the Community Health Center of Buffalo, Inc., opened its doors at 465 Grider Street, Buffalo, New York on the grounds of the Erie County Medical Center Since 1999, the Community Health Center of Buffalo, Inc. (CHCB) has been one of the leading health care providers in Buffalo, dedicated to providing a safety net to the uninsured and under insured by offering comprehensive primary care services in medicine and dentistry to a diverse population. We're more than just a health care center. We are part of the Community! We have been caring for our neighbors, your friends and family for over 18 years. We are here for you when you are sick and well! Providing You with Patient-Centered Medical Care We believe it is important for each patient to take control of their health and wellness by establishing a relationship with a primary doctor because it helps in the early detection of life threatening diseases. We strive to not only treat your illness during a visit but provide you with the information needed to manage any chronic disease or condition you may have like diabetes or high blood pressure. In December, 2018, we added physical therapy to our menu of service offerings. Our physical therapy space offers state of the art equipment and highly qualified physical therapy staff.

Filling a Health Care Need In 2010 the Community Health Center of Buffalo, Inc. opened a satellite location in Niagara Falls, New York to provide much needed primary care to the Niagara Falls community. In 2014, we moved from the Mizer Building on 10th Street in Niagara Falls, to a brand new site at 2715 Highland Avenue. The Community Health Center of Niagara (CHCN) is a one stop shop offering adult and pediatric primary care and dentistry, family planning. HIV testing and primary care, mental health counselling, as well as providing care for our LGBTQ community. Quest Laboratory and WNY Imaging are located in our building and our pharmacy is set to open sometime in 2018. The Community Health Center of Niagara is the first Federally Qualified Health Center in Niagara Falls. In the fall of 2011 the Community Health Center of Buffalo moved from our 465 Grider Street site to a beautiful 4 story modern facility at 34 Benwood Avenue, Buffalo, New York (the former St. Frances Nursing Home). We offer adult and pediatric primary care, dentistry, HIV testing and primary care, mental health counselling, as well as providing care for our LGBTQFI community. This location offers in-house laboratory services through Quest Labs as well as a 340b pharmacy where patients can purchase prescriptions at reduced prices

Our Lockport, New York site is located at 38 Heritage Court where we offer adult and pediatric primary care, HIV testing and primary care, family planning, mental health counselling and help for the LGBTQ community. Lockport patients needing dental services are referred to our Niagara Falls site.

In 2015, we opened our Cheektowaga, New York site at 934 Cleveland Drive where we offer adult and pediatric primary care, HIV testing and primary care, family planning, mental health counselling and help for the LGBTQ community. Cheektowaga patients needing dental services are referred to our Buffalo site.
