It has been nearly 18 years since we first opened our doors. I want to thank St. Lawrence Parish for their support over the years. Effective today, the clinic is closed. Record requests should be sent to 1520 E Delavan Ave., Buffalo, NY 14215. Vaccine records will be sent to out-of-state providers upon written request. In-state providers can obtain records by accessing NYSIIS.

We are open Monday, May 15 from 9am to noon for school, sports or camp exams and working papers physicals. Our vaccine supply is limited. We do not have chicken pox, TDaP, or Hepatitis B
