Tip for Hospice Nurses: The Centers for Medicare & Medicaid Services (CMS) recently announced that they will be intensifying their focus on the appropriateness of Continuous Care (CC) and General Inpatient Care (GIP). This implies an increase in ADRs/RAC audits and a heightened scrutiny of our documentation.

So, today I am discussing CC documentation in more detail.

CC documentation should not only validate hospice eligibility but also clearly illustrate the patient's need for CC and the specific skill you are providing. While documentation is straightforward when we initially start CC, as we are actively addressing the patient's symptoms, it becomes more challenging when the symptoms are managed and we are in the monitoring phase.

Here are some helpful tips for documenting during this period:

1. Document at least every 2 hours, and make each note unique.

2. Note 1: start with “Due to the interventions provided by hospice of ___” (list the interventions), then describe what the pt looks like now vs what they looked like before the interventions.

3. Note 2: Reverse the order of Note 1 by describing the patient's state (e.g., lying in bed calm and peaceful), the absence of certain symptoms, and the effectiveness of the hospice intervention.

4. Note 3: Document a system assessment that supports eligibility. Examples of this documentation can be found in my documentation books, and very soon the DAHN App.

5. Note 4: We provide a lot of education during CC. Document one area of education provided.

6. Note 5: Begin the cycle again with Note 1.

Documenting in this pattern will provide several hours of meaningful, unique entries.
