Insurance can be a frustrating part of healthcare for patients & doctors alike.

A few years ago, we dropped MediCal as an insurance, which was frustrating for many of our patients who rely on this coverage. The reason why we dropped it was that their $42.50 reimbursement for an exam and $19.90 for a pair of glasses, didn’t really come close to covering our expenses while leaving some profit at the end.

The sad truth is that many vision insurance plans, like VSP, haven’t increased their reimbursements to doctors in DECADES! Ever wonder why you may struggle to find an optometrist or dentist to take your vision or dental insurance? It’s usually because the reimbursements are so low, it’s extremely difficult to operate a successful practice on those payments. Those who stay open, frequently end up sacrificing quality for conveyer belt visits, seeing as many patients as humanely possible to somehow make enough money.

It’s not a great way to provide care. And it’s a shame that insurances don’t adjust reimbursements as prices increase every year & premiums for insurances do as well.

Doctors usually have very little negotiation power either to change this as we’re not their customers, but their product.

Nonetheless, the tireless effort of optometrist has now resulted in a pay increase for MediCal patients, and a quiet significant one - almost triple of previous payments. This is a fantastic development for anyone on MediCal, as this will encourage many doctors to become providers!

Changes are not supposed to go into effect until 2026, so we will wait and watch if they’ll follow through. But this is exciting news to hear.

If we’re lucky vision plans will feel pressured to follow suit.

UPDATE: We are no longer searching for additional team members, and greatly appreciate everyone who has submitted a resume, spread the word or expressed support. It seemed like our post disappointed some people for not paying comparable wages to the fast-food industry now.

We feel that every hard-working person should earn enough money to pay for the essentials of life, and we’re genuinely happy for every person hustling, who earns good money. We would love to pay better than McDonald’s, because we do truly value the people who help us serve our community.

However, what some may not realize is that despite being healthcare providers, we’re still a small business as well. Unlike our community health clinics, we don’t receive any government grants, and we’re a small fish in the health care ocean, which dramatically decreases our negotiating power to lower prices on goods. This is why Walmart or Costco can offer glasses, for example, at prices we can’t. Because we also rely primarily on insurance payments as our source of income, we cannot simply roll over an increase in cost to the consumer. McDonald’s may pay their employees $20/hr and can off-set that by simply increasing the cost of each burger by $0.50 for example. We don’t have that option. When we bill your insurance, the amount they will pay us is already determined in contracts. And those fees are usually 50-70% of what we charge. Insurance companies have actually decreased reimbursements (their payments to providers) for some services, all while cost for everything goes up. VSP, the vision plan we accept (it’s the best paying plan), has not increased payments to providers in decades! And again, because we’re a small fish, negotiating with insurance is utterly fruitless. Because we’re capped in a way on what we can earn, rising cost of goods and increasing labor cost, certainly put the pressure on us. You may say we can just take a pay cut, to pay better. This is also true. But we also have loan payments (that OD degree wasn’t free, neither was the purchase of the practice, the building or our state-of-the-art equipment) that need to be paid off still, as well as the reality that we also have kids to feed and doctor bills to pay, just like everyone else. In short, things may not always be as simply and straightforward as people may think.

The solution? We’re not sure. We’re healthcare providers, not politicians. But we will keep working on running a great practice that serves you well, and this will hopefully allow us to steadily keep improving pay & benefits for our employees. They’re wonderful, caring & hard-working people, who certainly deserve it. And if you want support our efforts, we certainly appreciate you coming to us for your care instead of the big chains!

Happy not quite so new New Year’s to everyone! I was reading how a fellow OD had a 7-year old sent to his practice from a rural ER for persistent, bad headaches. The doc found internal swelling of the eyes, sent the kid off promptly to a fully equipped hospital, where a brain tumor was found and removed. This is not a common cause for headaches, obviously. But it reminded me how your eyes are maybe not exactly a window to your soul, but certainly a window to the health of not only your eyes, but your whole body as well (see the attached article).

If you’ve come into my practice already this year for an eye exam, you may have noticed that we’ve added a new screening image to our routine exams, costing you $34 (insurance will not cover these images). I realize how tough the economy is on many, and how expensive healthcare can be. But I want to share how beneficial this new imaging is in providing the best possible care.

My goal as your doctor is to catch any issues as early as possible. Often, the sooner a health issue is found, the better and often less costly it is to treat or fix the issue. The Optomap is a piece of equipment that takes an image of your retina, and is excellent at getting into the periphery of your eye, which is often harder to see. For me, this essentially means that where I was previously peaking through a peep hole into your eye, the door is now wide open, and I can see everything! When I dilate, I can see about 80% of your eye. The Optomap can get similar results with its imaging. The image combined with a dilated look gets me to see essentially every area of your eye.

Just in the 3 weeks we’ve taken pictures now, I’ve already detected several serious issues, and one issue I couldn’t find just with dilation, even after seeing it on the image. I was only able to see it on the image we took. I would have missed this, and it was a more serious eye issue.

We’ve had some patients ask if they have a choice getting this image taken. Of course! You can say no to anything I do. However, I try to establish a standard of care that I want to give. I don’t want to miss something that could affect your health or vision long term. Taking these images is similar to getting x-rays at the dentist. You could say no. But the dentist can only do so much with their bare eyes. Same for me. Because I know cost can be a hurdle, we’ve tried to keep the cost low. Typically the charge (even at Costco) is $39 and upwards. I’m hoping by keeping it a bit lower, it’ll make it a bit more accessible to you. I’m excited to have this equipment to improve my ability to give you the best possible care! You don’t need to be excited about the $34. I know that part is no fun. Hopefully though you’ll be excited to know we’re truly reducing the chance of missing anything, and greatly improving my ability to detect any issues as early as possible. That’s definitely exciting to me.

Today was our last day seeing patients for the year, and it was quite the day. From getting delayed with car troubles in the morning, to having a new technician train (a tech training tends to slow me down, because they are making mistakes or are not quick enough yet) to having an employee out to have a baby (congrats!!!), to having a few patients with more complex needs than fit into the scheduled time slot, to a rep wanting to meet with me during lunch to patients wanting to see my face when opening the amazing gift they brought - today felt long, stressful, a bit chaotic and unfortunately some patients had to wait longer than they should. But I’m grateful for the wonderful community we serve - your patience, kindness and humor. And I’m grateful for our staff working so hard trying to make sure everyone is taken care of, even when we’re short staffed. They truly work so hard and I’m glad to have such a fabulous team.

I hope everyone has a wonderful Christmas and lovely New Year. We will still be open tomorrow and then will be closed until January 2, 2024.

We have an exciting announcement. We’re doing a $500 Give-Away. How does it work? If you have had a full eye-exam this year, you can come into our office and fill out a little paper to nominate one family or individual. The nominee needs to live in the intermountain area! You get to nominate another person or the same person also for each full pair of glasses you purchased this year.
We will draw a lucky community member on 12/20 at noon.
Why are we doing this? It takes a village to build community. Because of your support of our business, we have been able to offer steady employment to members of the community, and have been able to support our own family. In an effort to keep our patient's informed of all things eye related, we wanted to let you know of a recall on some artificial tear products. Announced on Feb. 2nd 2023, Global Pharma Healthcare announced a recall on EzriCare artificial tears and Delsam Pharma artificial tears, due to possible contamination. The two drops pictured are ones to pay close attention to. If you have these drops, dispose of them and don't use them. If you have been using them and you have any eye pain, discharge or vision concerns please contact our office. Please rest assured no other drops are involved in this recall.
