At SWMTCHC, we believe in whole-person care. As a Care Manager, you’ll be the heart of our mission—connecting patients with the resources they need to thrive. You’ll work closely with our primary care teams to assess social drivers of health, build trusting relationships, and help improve outcomes for a diverse and vibrant patient population.

What You’ll Do:

Serve as a point of contact for your patient panel, building strong, supportive relationships.
Assess patients for Social Drivers of Health and connect them with internal and external resources.
Collaborate with care teams to ensure timely, compassionate, and effective communication.
Help patients navigate healthcare systems, schedule appointments, and complete paperwork.
Maintain accurate documentation and uphold the highest standards of confidentiality and professionalism.
Contribute to specialty areas such as Pediatrics, Healthcare for the Homeless, or Mobile Van services.

What We’re Looking For:

Education: Degree in social work, public health, human services, or related field preferred. Relevant experience may substitute.
Experience: Prior work in healthcare or case management preferred. Experience with vulnerable or high-needs populations is a plus.

Skills & Traits: Solution-focused and compassionate
Excellent communicator and active listener
Tech-savvy and eager to learn new systems (EPIC experience a bonus!)
Strong organizational skills and ability to prioritize
Knowledge of Motivational Interviewing and Maslow’s Hierarchy of Needs
