It is sports season! We have the MHSAA Sports Physical Form located on our website; you can download, fill out, and send back to us. You can also pick up the form in our office!

Your child needs to be updated on their Yearly Physical or Well Child Visit to be able to complete the Sports Physical Form. Make sure to get them on the schedule soon be able to participate in fall sports.

Parents, MTP has started a text reminder program for scheduled appts!
Parents will get a text reminder a week before their child's scheduled appt, and 24 hours before the appt time. When parents receive a text from us, please confirm the appt time through the text. If you need to cancel or move the appt, please call our office at 231-779-9700.
If parents are not receiving a message at these scheduled times, please give our office a call so we can confirm a good contact number for your child's profile.
