At Central Alabama Wellness, we provide a safe, structured, and supportive environment for individuals on their journey toward mental wellness.

✨ What You'll Do:

Provides Case Management services to the seriously mentally ill population enrolled in Permanent Supported Housing program.

Serves as liaison with other agencies (i.e., Probate Office, state institutions, local hospitals, DHR, etc.) in identification and coordination of services for SMI referrals.

Develops and implements service plans, which are individualized and directed toward client service needs
