Camden Family Health is currently hiring a full-time case manager to work with our Office Based Medication Assistance Therapy Program. The applicant will work in a manner consistent with Camden on Gauley Medical Center’s mission, goals and standards of conduct and care
Essential Job Functions:
• Organizing and scheduling random drug screens and prescription verification
• Assure drug screen tests are in stock and quality tests are performed ready to use
• Insurance authorizations
• Create and update spread sheets for groups
• Schedule medical and behavioral health appointments for MAT patients
• Schedule follow up BH appointments for MAT patients for required appointments to meet state requirements
• Refer patients for inpatient treatment
• Intake interviews, and confirm support group meetings
• Provides discharge services and active follow-up to engage the client in appropriate services
• Attends treatment team meetings, and maintains contact with referral sources (e.g., Probation, DSS, etc.) as needed.
• Ability to engage in therapeutic communication with patients using motivational interviewing, supportive therapy, etc.
• Ability to function in the Electronic Medical Records by completing documentation medications and patient care
• All other duties as assigned
Education, Training and Experience:
• Education and Training: Bachelor’s degree in social service field and 2 + years’ experience. Experience may be accepted in place of degree.
• Additional training provided
• Current WV driver’s license proof of vehicle is required. Travel may be required to all COG sites as assigned. Travel may be required between clinic locations
HOURS PER DAY OR WEEK:
A 40-hour work week is required for full-time employment, occasional overtime
may be required.
