CNY Elderplanning is a geriatric care management agency that has served the elderly and their families for almost 20 years. We have a thorough knowledge of community resources that enable an elderly person to remain comfortably at home. A care manager will do an in-home assessment to determine needs, and keeping the client's preferences in mind, will offer a comprehensive plan to put services into place. We provide ongoing monitoring and care coordination, and act as a family liason. If relocation becomes necessary, CNY Elderplanning will assist with that process, always keeping the client's wishes and goals in mind.

A care manager will suggest a plan that might include personal care, companion care, housekeeping, transportation, and bill paying. We accompany some clients to medical appointments where we take notes on medical instructions. We are available to represent our clients in the Medicaid application process if that becomes necessary.
