We are accepting new referrals. Stop in for a tour and meet our team. We accept those who have Home and Community Based Waiver (HCBW), Support for Community Living (SCL), Michelle P Waiver(MPW), certain VA coverages, some Long Term Care policies and private pay. Active Day of Campbellsville offers skilled nursing services, assistance with activities of daily living, nutritional meals/ snacks, exercise program, transportation and a vast array of activities, crafts, community involvement and much more. We would love to talk to you about our services and how we can meet your loved one’s needs. Please stop by at 1500 East Broadway or give us a call at 270-464-9279.
