To begin our new year, weād like to remind patients of some important policies and procedures to ensure youāre receiving quality and timely care!
šAppointment Confirmations: Appointments must be confirmed 24 hours in advance. Unconfirmed appointments will be canceled and the patient will need to contact our office to reschedule.
šHealth History: All patients will be sent a new health history form to complete prior to their appointment. Please be sure to complete all parts to the form.
šInsurance Changes: Please send new insurance information prior to your appointment date. Insurance given on the same day of the appointment may result in delayed appointment times and/or the inability to give patients a proper treatment plan.
šOut of Network Polices: As a reminder, we are not in network with Anthem, Cigna or any of the KY Medicaid policies.
